About this file
This is a form template for environmental compliance at Dugway Proving Ground (DPG). Specifically, it is the Fugitive Dust Control Plan Form 05, designed to document fugitive dust emission control measures for staging areas where equipment, vehicles, or materials are parked, stored, or assembled during projects involving storage, hauling, and handling of aggregate material.
The form requires project-specific information including location(s), start and end dates, point of contact details, and a site description. Contractors must select at least one best management practice (BMP) from three critical mitigation areas: limiting visible dust opacity from vehicular operations (BMP 05-01 through 05-02), stabilizing staging area soils during use (BMP 05-03 through 05-04), and stabilizing soils at project completion (BMP 05-05 through 05-10). Available practices include vehicle speed/mileage restrictions, water application, chemical stabilizers, screened or washed aggregate, wind breaks, paving, and landscaping. When dust suppression is used, contractors must maintain a logbook documenting date, time, suppression type, amount in gallons, purpose, and application location. The form also establishes trackout prevention requirements per Utah Administrative Code R307-205-6(2), mandating that contractors prevent material deposit on paved roads except at designated sites and promptly clean roads if contamination occurs. Completed forms and applicable logbook entries must be submitted to the Environmental Program's Air Program Manager at boyd.roberts.civ@mail.mil upon operation completion or annually by calendar year-end, whichever occurs first.
