About this file
This is a template form for documenting fugitive dust control measures at Dugway Proving Ground (DPG), specifically addressing trackout prevention and cleanup of mud, silt, and soil deposited onto paved roads by vehicles and equipment. The form requires contractors to complete general information fields including location(s), start and end dates, point of contact information, and site description.
The form mandates selection of at least one best management practice (BMP) from designated mitigation categories. Dust prevention options include cleaning trackout at shift end (BMP 17-01 and 17-02), installing gravel pads with minimum specifications of 30 feet wide by 3 inches deep and at least 50 feet long (BMP 17-03), installing and maintaining wheel shakers (BMP 17-04) or wheel washers (BMP 17-05), restricting motorized vehicles to paved surfaces only (BMP 17-06), and installing cattle guards before paved road entrances (BMP 17-07). For routing exiting traffic, contractors must select either clearly establishing traffic patterns over trackout control devices (BMP 17-08) or limiting site accessibility through barriers on unprotected routes (BMP 17-09). The form references compliance with Utah Administrative Code R307-205-6(2) requiring contractors to prevent material deposition on paved roads and promptly clean any fugitive dust materials that are deposited. Completed forms must be submitted to the Environmental Program's Air Program Manager at boyd.roberts.civ@mail.mil upon operation completion or annually by December 31, whichever occurs first.
