About this file
This is an environmental compliance form template for fugitive dust control at Dugway Proving Ground (DPG), specifically addressing sand and gravel borrow pit operations and crushing of rock, soil, or construction debris. The form requires operators to document crushing activities and implement best management practices (BMPs) to control dust emissions in accordance with the DPG Fugitive Dust Control Plan (FDCP) and Utah Administrative Code R307-205-6(2).
Operators must complete general information sections including location, dates, point of contact details, and site description, then select at least one BMP from each of six critical mitigation areas: stabilizing surface soils for equipment and vehicle operations (three options including pre-watering, chemical stabilization, or paving); stabilizing materials before crushing (two options including pre-watering or moisture content testing); stabilizing material during crushing (three options including water application, opacity monitoring, or wind breaks); stabilizing material after crushing (four options including immediate watering, chemical stabilization, enclosure, or stockpile height minimization); reducing traffic impacts (three options); and minimizing transfer height. All crushing operations require logbook maintenance documenting date and amount of material crushed at minimum; if suppressants are used, additional entries must record date, time, type, amount in gallons, purpose, and application location. Operators must prevent trackout onto paved roads and clean roads promptly if material is deposited. The completed form must be submitted via email to the Environmental Program's Air Program Manager (boyd.roberts.civ@mail.mil) prior to activity commencement, with copies of completed forms and corresponding logbook entries provided upon operation completion or annually by calendar year end, whichever occurs first.

Dugway Proving Ground Fugitive Dust Control Plan Form 01 Sand and Gravel Borrow Pit Operations
Crushing of Rock, Soil, or Construction Debris Activity Description: The process of reducing the size of materials by physical impaction.
General Information
Location(s): Start Date: End Date:
Point of Contact: Email: Phone Number:
Description of site:
Fugitive Dust Emission Control Techniques Select at least one best management practice (BMP) for each critical mitigation area:
Stabilize surface soils where support equipment and vehicles will operate
BMP 01-01 Pre-water and maintain surface soils in a stabilized condition.
BMP 01-02 Apply and maintain a chemical stabilizer to surface soils. (Name of chemical: ) BMP 01-03 Pave operational area(s).
Stabilize materials before crushing BMP 01-04 Pre-water material.
BMP 01-05 Test material to determine moisture content and silt loading, crushing only material that is at optimum moisture content.
Stabilize material during crushing BMP 01-06 Apply water to stabilize material so as to maintain compliance with opacity standards and permit conditions.
BMP 01-07 Monitor opacity. Make adjustments to maintain compliance with opacity standards and permit conditions.
BMP 01-08 Install wind breaks or use enclosure.
Stabilize material after crushing BMP 01-09 Water crushed material immediately following crushing.
BMP 01-10 Apply and maintain a chemical stabilizer to crushed material. (Name of chemical: ) BMP 01-11 Maintain in enclosure.
BMP 01-12 Minimize height of stockpile.
Traffic BMP 01-13 Minimize vehicle miles.
BMP 01-14 Reduce truck traffic.
BMP 01-15 Reduce truck speed.
Transfer height BMP 01-16 Minimize transfer and drop point height.
Logbook Requirement All crushing operations require a logbook be kept, at minimum, it must contain: the date and amount of material crushed. If suppression is used, (i.e.
water/chemical suppressant), additional logbook entries are required, at minimum, entries must contain: the date, time, type of suppression used, amount (in gallons), purpose and location applied.
Trackout Prevention In accordance with Utah Administrative Code R307-205-6(2), any person(s) performing fugitive dust emission activities will to the maximum extent possible, keep material from being deposited onto any paved road other than a designated deposit site. Any such person(s) who deposits materials that may create fugitive dust on a paved road shall clean the road promptly.
*By signing you attest that you have read and understand the requirements of the DPG FDCP and will follow the BMPs you have selected.
Signature Date This form is to be completed and kept on record according to the requirements found in the Dugway Proving Ground (DPG) Fugitive Dust Control Plan (FDCP). Submit completed form in hardcopy or electronically, via email to Environmental Program’s (EP) Air Program Manager: boyd.roberts.civ@mail.mil prior to activity occurring. Upon the completion of operation(s) and/or annually at the end of each calendar year whichever occurs first, copies of form(s) and corresponding logbook entries (if applicable) shall be provided to EP, Air Program Manager.
