About this file
This is a Fugitive Dust Control Plan form template for hauling materials at Dugway Proving Ground. The form applies to transportation of aggregate materials to and from job sites and requires contractors to implement best management practices (BMPs) to control fugitive dust emissions.
Contractors must select at least one BMP from each of three critical mitigation areas: limiting visible dust opacity from vehicular operations (applying water/chemical suppressants or limiting vehicle mileage and speed), stabilizing materials during transport (using tarps or applying water prior to transport), and cleaning wheels and undercarriage of haul trucks (cleaning wheels or sweeping/watering haul roads). All aggregate hauling operations require a logbook documenting the date and amount of material hauled, and if suppressants are used, additional entries must record the date, time, type and amount of suppressant in gallons, purpose, and location applied. The form requires compliance with Utah Administrative Code R307-205-6(2) regarding trackout prevention, mandating that contractors keep material from being deposited on paved roads other than designated sites and promptly clean any roads where material is deposited. Completed forms and corresponding logbook entries must be submitted to the Environmental Program's Air Program Manager at boyd.roberts.civ@mail.mil upon operation completion or annually at year-end, whichever occurs first. The form requires contractor signature attesting to understanding and compliance with the Fugitive Dust Control Plan requirements.
