About this file
This is a federal environmental compliance form required by Dugway Proving Ground (DPG) for managing fugitive dust emissions during sand and gravel borrow pit operations, specifically for screening of rock, soil, or construction debris activities.
The form requires operators to complete general information fields including location, dates, and point of contact details, then select at least one best management practice (BMP) from five critical mitigation areas: stabilizing surface soils where equipment operates, pre-treating material prior to screening, stabilizing material during screening, stabilizing material after screening, and controlling transfer height. Available BMPs include water pre-treatment and maintenance, chemical stabilizers, paving, dust suppressants, wind barriers, and minimizing storage pile heights. Operators must maintain a logbook documenting the date and amount of material screened, with additional entries required if suppressants are used, including time, type, amount in gallons, purpose, and location applied. The form incorporates trackout prevention requirements per Utah Administrative Code R307-205-6(2), requiring operators to prevent material deposition on paved roads and promptly clean any contaminated surfaces. Completed forms must be submitted to the Environmental Program's Air Program Manager at boyd.roberts.civ@mail.mil prior to activity commencement, with copies of forms and logbook entries due upon operation completion or annually by calendar year-end, whichever occurs first. The signatory attests to understanding DPG Fugitive Dust Control Plan requirements and commitment to following selected BMPs.

Screening of Rock, Soil or Construction Debris Activity Description: The mechanical process which accomplishes a division of particles on the basis of size.
General Information
Location(s): Start Date: End Date:
Point of Contact: Email: Phone Number:
Description of site:
Fugitive Dust Emission Control Techniques Select at least one best management practice (BMP) for each critical mitigation area:
Stabilize surface soils where support equipment and vehicles will operate
BMP 02-01 Pre-water and maintain surface soils in a stabilized condition.
BMP 02-02 Apply and maintain a chemical stabilizer to surface soils. (Name of chemical: ) BMP 02-03 Pave operational area(s).
Pre-treat material prior to screening BMP 02-04 Apply a dust suppressant to material. (Name of chemical: )
Stabilize material during screening BMP 02-05 Dedicate water source to screening operation and apply water as needed to prevent dust.
BMP 02-06 Install wind barrier upwind screen as high as the drop point.
Stabilize material after screening BMP 02-07 Apply water to stabilize screened material and surrounding area.
BMP 02-08 Apply and maintain a chemical stabilizer to stabilize screened material and surrounding area.
(Name of chemical: )
BMP 02-09 Minimize storage pile height.
Transfer height
BMP 02-10 Drop material through the screen slowly and minimize drop height.
Logbook Requirement All screening operations require a logbook be kept, at minimum, it must contain: the date and amount of material screened. If suppression is used, (i.e. water/chemical suppressant), additional logbook entries are required, at minimum, entries must contain: the date, time, type of suppression used, amount (in gallons), purpose and location applied.
Trackout Prevention In accordance with Utah Administrative Code R307-205-6(2), any person(s) performing fugitive dust emission activities will to the maximum extent possible, keep material from being deposited onto any paved road other than a designated deposit site. Any such person(s) who deposits materials that may create fugitive dust on a paved road shall clean the road promptly.
*By signing you attest that you have read and understand the requirements of the DPG FDCP and will follow the BMPs you have selected.
Signature Date This form is to be completed and kept on record according to the requirements found in the Dugway Proving Ground (DPG) Fugitive Dust Control Plan (FDCP). Submit completed form in hardcopy or electronically, via email to Environmental Program’s (EP) Air Program Manager: boyd.roberts.civ@mail.mil prior to activity occurring. Upon the completion of operation(s) and/or annually at the end of each calendar year whichever occurs first, copies of form(s) and corresponding logbook entries (if applicable) shall be provided to EP, Air Program Manager.
