About this file
This is a Fugitive Dust Control Plan Form 14 template for construction and demolition trenching activities at Dugway Proving Ground (DPG). The form requires contractors to document trenching operations and select best management practices (BMPs) to control fugitive dust emissions during long, narrow excavations performed with track or wheel-mounted equipment such as excavators, shovels, backhoes, or trenchers.
Contractors must complete general information fields including location, start and end dates, point of contact information, and site description. For fugitive dust emission control, at least one BMP must be selected from three critical mitigation areas: presoak soils prior to trenching (BMP 14-01: pre-water surface), stabilize surface soils where equipment operates (BMPs 14-02 through 14-04: pre-water/maintain, apply chemical stabilizers, or limit mileage and speed), and stabilize soils after trenching (BMPs 14-05 and 14-06: apply and maintain water or chemical stabilizers on excavated soil). When suppression methods are used, contractors must maintain logbooks documenting the date, time, suppression type, amount in gallons, purpose, and application location. Additionally, contractors must comply with Utah Administrative Code R307-205-6(2) trackout prevention requirements by preventing material deposition on paved roads and promptly cleaning any materials deposited. Completed forms and corresponding logbook entries must be submitted to the Environmental Program's Air Program Manager at boyd.roberts.civ@mail.mil upon operation completion or annually by calendar year end, whichever occurs first.
