About this file
This is a template form for the Dugway Proving Ground Fugitive Dust Control Plan (FDCP) used for construction and demolition activities involving sawing and cutting materials. The form is designed to document and control fugitive dust emissions from activities such as cutting concrete, asphalt, block, or pipe at Dugway Proving Ground.
The form requires contractors to provide general information including location(s), start and end dates, point of contact details, and site description. Contractors must select at least one best management practice (BMP) from the available fugitive dust emission control techniques to limit visible emissions: either BMP 12-02 (water control to dust) or BMP 12-03 (vacuum collection). If water suppression is used, a logbook is required documenting the date, time, amount in gallons, purpose, and location of application. The form mandates trackout prevention in accordance with Utah Administrative Code R307-205-6(2), requiring contractors to prevent material deposit on paved roads except at designated sites and to promptly clean any roads contaminated with fugitive dust materials. Completed forms and corresponding logbook entries must be submitted to the Environmental Program's Air Program Manager at boyd.roberts.civ@mail.mil upon completion of operations or annually at the end of each calendar year, whichever occurs first. The form requires a signature attesting that the contractor understands and will comply with FDCP requirements and selected BMPs.
