VHAPM Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT

VHAPM Part 813.106 Page 1 of 1 Original Date: 08/30/17 Revision 02 Eff Date: 03/31/2020

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C252-20-AP-3856

1. Contracting Activity: Department of Veterans Affairs, Great Lakes Acquisition Center, NCO 12, 115 South

84th Street, Milwaukee, WI 53214. Requesting Activity: Department of Veterans Affairs, VISN 12, Clement J.

Zablocki VA Medical Center, 5000 W. National Avenue, Milwaukee WI 53295. Purchase Request (2237) number: 695-21-1-6940-0001.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Jeron Health Care Call System Provider 790 Nurse Call System is the only product that allows for a single

Nurse Call system for the Milwaukee facility. It cannot be expanded by other vendors or manufacturers and can only be expanded by the same vendor/manufacturer. Cost estimate: $35,000.00.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

Jeron Nurse Call System signals are sent back to head end equipment in Building 111. The backbone of the system has already been installed at great expense. This project’s expansion of the hospital’s Nurse Call system will be a modification of the existing system and not a new system. An incompatible system will require installation of new head end and backbone components – extremely cost prohibitive. Installation of a system that interfaces properly with the existing system in Building 111 will ensure patient safety because communications will be seamless. Installation of components that match the existing nurse call system minimizes maintenance costs and bench stock requirements. No other system will integrate with our existing

Nurse Call system.

4. Description of market research conducted and results or statement why it was not conducted:

No other system that offers a seamless integration with our existing Nurse Call system.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
