VHAPM Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT

VHAPM Part 813.106 Page 1 of 1 Original Date: 08/30/17 Revision 02 Eff Date: 03/31/2020

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C252-20-AP-3856

1. Contracting Activity: Department of Veterans Affairs, Great Lakes Acquisition Center, NCO 12, 115 South

84th Street, Milwaukee, WI 53214. Requesting Activity: Department of Veterans Affairs, VISN 12, Clement J.

Zablocki VA Medical Center, 5000 W. National Avenue, Milwaukee WI 53295. Purchase Request (2237) number: 695-21-1-6940-0001.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Puritan-Bennett (Beacon Medaes) Medical Gas Alarm system and medical gas outlets (Chemtron) are the only products that allows for a single medical gas delivery and alarm system for the Milwaukee facility. The existing medical gas system currently installed at VAMC Milwaukee cannot be expanded by other vendors or manufacturers. This system can only be expanded by the same vendors/manufacturers. The medical gas delivery and alarm systems are singular and has been recently upgraded throughout Building 111. Cost estimate: $11,500.00.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The signal(s) are sent back to the existing master alarm panels in Building 111 and 113 where they are monitored. A different system may require installation of new master panel equipment (cost ineffective) or may be incompatible (causing patient safety issues). Installation of a system that interfaces properly with the existing system in Building 111 will ensure patient safety because communications will be seamless, and furthermore, the staff is familiar with the medical gas alarm components. Additionally, installation of components that match the existing medical gas alarm system minimizes maintenance costs and bench stock requirements.

4. Description of market research conducted and results or statement why it was not conducted:

No other system will integrate with our existing Medical Gas Alarm system. Need to match existing system to ensure there are no interface problems and that system operation and maintenance is maintained to ensure patient and worker safety.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
