About this file

This is a Past Performance Questionnaire (PPQ) template designed for the VA Program Contracting Activity Central to evaluate contractor past performance for federal contract opportunities. The document serves as an evaluation tool to be completed by references or clients who have worked with an offeror on previous projects and incorporated into the offeror's proposal submission, counting toward the proposal page limitation outlined in the CON 100 Instructions to Offerors.

The PPQ requires contractors to provide detailed information in Section 1 including offeror name, address, point of contact details, work performed role, and project participation percentage, along with comprehensive contract data such as contract title, contract number, contract type, contract location, award date, acceptance dates, and pricing information (original implementation price, original contract price, and final contract price with modifications). Section 2 requires an evaluator (client or reference) to assess the contractor's past performance across seven evaluation categories: Quality (ability to meet design and construction standards), Schedule/Timeliness of Performance (compliance with delivery schedules), Communication (cooperation and responsiveness), Management/Personnel/Labor (effectiveness of management and personnel continuity), Cost/Financial Management (ability to meet contractually agreed prices), Subcontract Management (managing subcontracting efforts and resolving issues), and Summary (overall performance rating and likelihood of rehiring). Evaluators assign adjectival ratings of Outstanding, Above Average, Satisfactory, Unsatisfactory, or Not Applicable with supporting comments. The form includes signature and date blocks for the evaluator and specifies that the ratings definition page will be removed prior to proposal evaluation and will not count toward the proposal page count.
