AMENDMENT 0003

The purpose of Amendment 0003 is to:

1. Revise SECTION B – SUPPLIES OR SERVICES AND PRICES, B.3 CONTRACT LINE

ITEMS, CONTRACT EXHIBIT LINE ITEMS AND SUBCLINS to reflect the following:

FROM:

B.3 CONTRACT LINE ITEMS, CONTRACT EXHIBIT LINE ITEMS AND

SUBCLINS

c. The Schedule of Recurring work (CLINs 0001, 0003, 0005, 0007, 0009, 0011, and 0013), and the Schedule of Non-Recurring work (CLINs 0002, 0004, 0006, 0008, 0010, 0012, and 0014), will be used as the basis for deductions in accordance with the FAR Clause 52.246-4 Inspection of Services - Fixed Price (Aug. 1996).

TO:

B.3 CONTRACT LINE ITEMS, CONTRACT EXHIBIT LINE ITEMS AND

SUBCLINS

c. The Schedule of Recurring work and the Schedule of Non-Recurring work will be used as the basis for deductions in accordance with the FAR Clause 52.246-4 Inspection of Services - Fixed Price (Aug. 1996).

2. Provide Responses to RFIs with “Pending” responses and missing responses from Amendment 0002.

3. Provide Responses to additional RFIs

4. The proposal due date remains unchanged at Monday, June 14, 2021 at 12:00PM Pacific

Daylight Time (PDT).

5. All other terms and conditions remain unchanged.

NOTE: THE OFFEROR MUST ACKNOWLEDGE RECEIPT OF ALL AMENDMENTS PURSUANT TO THE

METHOD NOTED ON ITEM 11. OF STANDARD FORM (SF) 30.

A. The following are responses to Requests for Information:

RFI #57: Pages 81 through 128 appear to be duplicates of portions from pages 5 through 81 Please clarify.

RESPONSE: Pages 81 – 128 are duplicates. It is the way the Government’s contracting system included the text the Contract Specialist added as a document to the system, which started at Page 5. It was a system generated formatting issue that created the duplication.

RFI #64: The Safety Narrative (limit 2 pages total) is listed twice; first on pages 2 through 4 (numbered items 6a, 6b, and 6c) and again on page 5 (numbered items 7a, 7b, and 7c). The items listed for discussion in Block 6 (pages 2 through 4) differ from those required in Block 7 (page 5). Is the Offeror to complete two different Safety Narratives, each limited to no more than 2 pages each? Please clarify.

RESPONSE: Corrected EXHIBIT D Factor 3 Safety has been provided with this Amendment 0003. The Offeror is to complete one Safety Narrative. Numbered items 7a, 7b, and 7c have been deleted.

RFI #65: The last item on page 1 is numbered 3.c. On page 2, the first item is numbered 6.a. Are there items/sub-items (4 and 5) missing? Please clarify.

RESPONSE: Corrected EXHIBIT D Factor 3 Safety has been provided with this Amendment 0003.

RFI #66: Section M requires the Safety Narrative to address the following three requirements: ”

a. Offeror describes a viable SMS that addresses elements; such as Management/Leadership involvement, Employee involvement, Hazard prevention, Hazard control, Worksite analysis, and Safety and health training, to include the standard(s) used to benchmark the SMS. b. Offeror describes a methodical process of evaluating subcontractor’s safety performance in their selection process. c. Offeror describes a logical management plan to hold both themselves and their subcontractors accountable for adhering to the safety requirements of the contract.” These three items seem to pair with the requirements of Block 6 (pages 2 through 4) of Exhibit D, but not with Block 7 (page 5) of Exhibit D. Please clarify.

RESPONSE: See RESPONSE to RFI #64.

RFI #109: Exhibit D, Factor 3, Safety does not align with the requirements listed in Section M, Evaluation Criteria.

There is no identified area on Exhibit D for the required TCR rate i information. Also, the form states that the Technical Approach to Safety should not exceed 2 pages, but the form fields for this section exceed 2 pages. Could the Government please release an updated Exhibit D, correcting these discrepancies?

RESPONSE: See RESPONSE to RFI #65.

RFI #111: RFP. B.3. c. “The Schedule of Recurring work (CLINs 0001, 0003, 0005, 0007, 0009, 0011, and 0013), and the Schedule of Non-Recurring work (CLINs 0002, 0004, 0006, 0008, 0010, 0012, and 0014), will be used as the basis for deductions in accordance with the FAR Clause 52.246-4 Inspection of Services - Fixed Price (Aug.

1996).” Request clarification from the Contracting Officer if the unit price or total price on these CLINs will be used for deductions when the Government determines that the work does not comply with the Performance Requirements Summary. The RFP only shows 5 CLINS so request clarification on where the identified CLINS are found.

RESPONSE: Section B.3. has been revised. See above. The Schedule of Recurring Work and the Schedule of Non- Recurring Work will apply to each individual Task Order as required.

RFI #120: RFI #57 was not answered. Please provide answer.

RESPONSE: See RESPONSE to RFI #57 above.

RFI #121: RFI #s 64, 65, 109, and 111 all say “Response Pending.” When will they be answered and solicitation amended with these answers?

RESPONSE: All RFIs are being responded to in this Amendment 0003 that is being issued today, June 3, 2021.

RFI #121: RFI #s 64, 65, 109, and 111 all say “Response Pending,” and the change in proposal requirements, we request additional time past the 14th June for the proposal submission date to allow the Government to answer the responses pending and for the bidders to adjust their proposal to the new requirements. Suggest June 21st as the new date.

RESPONSE: The Proposal Due Date remains unchanged at Monday, June 14, 2021 at 12:00PM Pacific Daylight Time

(PDT).
