About this file

This document is a Representation Regarding Certain Telecommunications and Video Surveillance Services or Equipment form (REV 10/20), implementing Section 889 of the John S. McCain National Defense Authorization Act for Fiscal Year 2019. The form requires offerors to disclose whether they provide or use covered telecommunications equipment or services, with specific prohibitions on procuring or contracting with entities using such equipment from specific manufacturers deemed to pose potential security risks.

The form mandates that offerors represent whether they will provide covered telecommunications equipment or services to the government, and whether they use such equipment or services. If an offeror responds affirmatively to either representation, they must provide detailed disclosure information, including the entity that produced the equipment, a description of the equipment or services, and an explanation of the proposed use. The provisions aim to prevent federal agencies from procuring or working with entities using telecommunications equipment or services from certain manufacturers that may pose national security concerns.
