About this file
This is a Utility Work Schedule document issued by the Florida Department of Transportation (FDOT) in coordination with Florida Public Utilities Company for utility relocation and adjustment work associated with the Veterans Memorial Parkway Extension project in Volusia County, Florida. The document outlines the requirements and timeline for Florida Public Utilities to relocate or adjust three separate gas main facilities within the project limits: a 6-inch HDPE main gas pipe between stations MLK 11+80 and MLK 12+80 that crosses existing facilities; a 6-inch HDPE main gas line near and parallel to a storm drain between stations MLK 15+00 and MLK 16+50; and a 4-inch HDPE main gas crossing between stations MLK 16+80 and MLK 26+00. All three utility work activities are dependent on contractor notification and are scheduled to occur during FDOT project construction, with each activity assigned one consecutive calendar day for completion. The utility company project representative is Fernando Van-Leeuwen (561-389-2338) and the field representative is Matt Ryan (561-531-3847).
The utility work schedule document specifies several special conditions and constraints that must be observed during construction. Florida Public Utilities representatives must be present when construction occurs within five feet of the buried gas main, with two weeks' advance notice required. The road contractor is responsible for performing staking with offset and elevation measurements and for completing clearing and grubbing activities prior to Florida Public Utilities' work. In the event of any damage, disturbance, or puncture to gas facilities, the contractor must immediately notify the right-of-way owner, Florida Public Utilities, and the local Fire Department. An emergency contact number of 1-800-427-7712 is provided for nights, weekends, and holidays. The utility work schedule is compatible with FDOT plans dated December 14, 2016, and any deviation from those plans may void the schedule, though the utility company may negotiate a new schedule upon notification of plan changes by FDOT.

The UAO shall obtain a utility permit and comply with requirements of the 2017 Utility Accommodation Manual (UAM) for all work done under this utility work schedule.
