About this file
This is a Final Asbestos Abatement Report Template for Ellington Field Joint Reserve Base, prepared in accordance with Texas Department of State Health Services (TDSHS) regulations and federal Environmental Protection Agency (EPA) standards. The template outlines the documentation requirements for asbestos abatement projects conducted at the facility located at 14657 Sneider Street, Houston, Texas 77034. The report serves as a completion document summarizing project duration, work performed, air monitoring results, and regulatory compliance. The asbestos abatement scope includes removal of approximately 1,350 square feet of asbestos-containing floor tile and mastic. All work must comply with Texas Health and Human Services regulations under Texas Administrative Code Title 25, Part 1, Chapter 296, and federal Code of Regulations Title 40, Chapter 1, Part 61 (NESHAP standards).
The template requires submission of comprehensive documentation including TDSHS notifications submitted at least ten days prior to abatement commencement, Phase Contrast Microscopy (PCM) air samples analyzed according to NIOSH Method 7400, chain of custody forms, air monitoring field forms, daily project notes, TDSHS certifications and licenses for prime and subcontractors, before and after photographs, and signed waste manifests. Air monitoring must demonstrate that all area samples collected outside the work area remain at or below 0.01 fibers per cubic centimeter and that all personal samples remain below OSHA Permissible Exposure Limits (PEL of 0.1 f/cc TWA and 1.0 f/cc STEL). Waste materials must be manifested and transported by licensed waste transporters to approved landfills, with the military base designated as the generator. A TDSHS-licensed Asbestos Air Monitoring Technician must conduct continuous monitoring, final visual inspection for cleanliness, and daily air sampling throughout the project duration.
