About this file
This is a hazardous material tracking form for Ellington Air Force Base (EF) under the Air Force Environmental, Safety, and Occupational Health Management Information System (AF-EESOHMIS). The form requires contractors to report hazardous material usage data to support environmental compliance and regulatory requirements. Contractors must submit this form monthly for contracts exceeding six months in duration, or at the start and completion of work for purchase orders or contracts under six months. The form collects information including contractor name, AF-EESOHMIS shop code, contract number, and work locations to maintain accurate on-base tracking of hazardous materials.
Submission of the completed form is required to the Government Representative, who forwards it to the HAZMART office for records management and compliance purposes. This documentation requirement applies to all contractors working on Ellington AFB and is mandatory under applicable Environmental Laws and Regulations. The form serves as an administrative control mechanism to monitor contractor hazardous material handling activities and ensure adherence to environmental protection standards throughout the contract period.
