About this file

This document is an attachment to a contract specification for Ellington Field in Houston, Texas, establishing safety policies and procedures required of contractors performing work at the installation. The specification mandates that contractors submit qualifications documentation including a Statement of Qualification for a land surveyor or civil engineer, a contractor health and safety plan, a construction hazard plan, and a temporary utility plan. All work must comply with OSHA 1926 regulations (29 CFR 1926). Contractors must designate a qualified, OSHA-trained safety representative who remains on-site during all work activities and is responsible for overall accident prevention supervision and jobsite safety inspections. The contractor must develop a Job Hazard Analysis and Job Safety and Health Plan for presentation at the preconstruction conference.

The Health and Safety Plan must meet specific minimum requirements including written hazard analysis for each task, employee training documentation with weekly safety briefing records, personal protective equipment specifications, medical surveillance provisions, air and environmental monitoring procedures, site control coordination with the installation, decontamination procedures for chemical materials, emergency response procedures, confined space entry protocols, spill containment provisions, and accident notification and reporting procedures. A safety and health supervisor meeting 29 CFR 1926 supervisor training requirements must verify adherence to the plan in writing. Contractors must also submit a Temporary Utility Plan addressing electrical, water, sanitary facilities, and other utilities, all requiring Project Manager approval and coordination. Monthly safety meetings with all personnel and subcontractors are mandatory, with meeting minutes and attendance rolls submitted to the Project Manager at each progress meeting. Any discovered hazards such as transite pipe or contaminated soils must be addressed through an Emergency Response Plan, and Safety Data Sheets for all materials must be maintained on-site.
