Assessment

This Task Assignment authorizes GHD Services, Inc. (GHD) to conduct site assessment activities. Specific subtasks are detailed below:

Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.

Subtask 2: Groundwater Sampling Initially, GHD will coordinate site access with the current property owner and adjacent property owner. Additionally, GHD will negotiate subcontract agreements for drilling waste disposal services. Following initial contracting activities, GHD will mobilize to the subject site and sample up to eleven (11) existing monitor wells. The sampling will be conducted in general accordance with FDEP SOPs and all samples will be analyzed for the presence of volatile organics by EPA Method 8260. In addition, up to four (4) well samples will be analyzed for the presence of petroleum compounds (acenaphthene, acenaphthylene, 1-methylnpahthalene, 2- methylnaphthalene, and naphthalene by EPA method 8270SIM and total recoverable petroleum hydrocarbons (TRPH) via State Method FL-PRO). Upon receipt, the analytical data will be evaluated and DEP will be provided a summary with any recommendations for changes to the proposed scope.

Subtask 3: Soil and Groundwater Sampling Soil sampling and groundwater profiling will be conducted in order to establish the extent of soil and groundwater impacts both vertically and horizontally. Soil sampling will include advancement of soil borings within and in the immediate vicinity of the former drycleaner with hand operated equipment and through the use of direct-push technology (DPT). Modified Active Gas Sampling (MAGS) will also be conducted as part of the soil assessment. One continuous soil boring will be completed to establish site lithologic conditions. The initial scope of work includes groundwater profiling at seven (7) exterior locations. All groundwater samples will be submitted to a Florida-certified laboratory for volatile organic analysis by EPA Method 8260. The initial soil sampling will involve the advancement of twelve (12) shallow soil borings within and near the former drycleaner facility. All soil samples will be submitted to a fixed laboratory for analysis for the presence of volatile organic compounds (VOCs). Following soil sampling, piezometers will be installed at ten (10) locations. The piezometers will be constructed with 2-inch PVC well screens across the vadose zone. Following installation, MAGS will be conducted and soil gas samples will be gathered for field screening and fixed laboratory analysis for the presence of VOCs by Method TO-15.

During the same mobilization, permanent monitor wells will be installed. At this time, up to six (6) shallow and one deep well are planned for installation. One of the shallow wells will be installed within the facility using hand operated equipment. Well locations and construction details will be provided to the Department for approval prior to installation.

Following installation, the wells will be surveyed and sampled in accordance with FDEP SOPs. Groundwater samples will be submitted to a fixed laboratory for VOC analysis by EPA Method 8260.

Subtask 4: Surveying/IDW Disposal Following all field activities, a surveyor will be subcontracted to develop a site plan and survey elevations of all sample locations. Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Subcontractors for IDW disposal were chosen on a lowest-cost basis.

Subtask 5: Site Assessment Report Upon receipt of analytical data, a Site Assessment Report (SAR) will be prepared. The SAR will include a summary of field methodology, field screening data, and laboratory analytical results. The SAR will also include conclusions and recommendations. The recommendations may include the use of active remediation to mitigate identified impacts.

GHD and the FDEP confirmed on May 4, 2023, that an Access Agreement with the current real property owner of the aforementioned site has been executed.

This work will be conducted in line with the scope of work included with GHD’s proposal, received May 4, 2023. Documentation of the costs is included in the attached CEF, Level-of-Effort sheets, and proposal letter. The final deliverable due date is November 24, 2023, and the completion date for the task assignment is February 23, 2024.

The final cost total for Task Assignment DC037B is $83,347.90.

COST PROPOSAL FOR TASK DC037B

<Courtesy Cleaner/FDEP ERIC_5435>

< Site Assessment >

1. INTRODUCTION: <DISCUSS SCOPE OF WORK, DATES >

GHD Services Inc. (GHD) submitted a Site Assessment Workplan for the Courtesy Cleaners site located in Sarasota, Sarasota County, in February 2023. The Site Assessment Workplan detailed an approach to determining the extent of contamination in soil and groundwater both vertically and horizontally. Task DC037B includes initial site assessment activities including soil sampling, modified active gas sampling (MAGS), discrete groundwater profiling using direct push technology (DPT), monitor well installation, and groundwater sampling

Subtask 1: Project Management Subtask 1 includes all tasks associated with project management of the site assessment activities at the Courtesy Cleaners site.

In addition, up to four (4) well samples will be analyzed for the presence of petroleum compounds (acenaphthene, acenaphthylene, 1-methylnpahthalene, 2-methylnaphthalene, and naphthalene by EPA method 8270SIM and total recoverable petroleum hydrocarbons (TRPH) via State Method FL-PRO). Upon receipt, the analytical data will be evaluated and DEP will be provided a summary with any recommendations for changes to the proposed scope.

Subtask 3: Soil and Groundwater Sampling Upon Department approval, GHD will mobilize with the selected direct-push contractor. Soil sampling and groundwater profiling will be conducted in order to establish the extent of soil and groundwater impacts both vertically and horizontally. Soil sampling will include advancement of soil borings within and in the immediate vicinity of the drycleaner with hand operated equipment and through the use of direct-push technology (DPT). Modified Active Gas Sampling (MAGS) will also be conducted as part of the soil assessment. One continuous soil boring will also be completed to establish site lithologic conditions.

The initial scope of work includes groundwater profiling at seven (7) exterior locations. At each location, direct-push tooling will be advanced to the desired depths and discrete groundwater samples will be gathered for fixed laboratory analysis. For planning purposes, it is assumed that discrete groundwater samples will be gathered from the surface of the groundwater, 10, 20 feet, 30 feet, 40 feet, and 50 feet below land surface. All groundwater samples will be submitted to a Florida-certified laboratory for volatile organic analysis by EPA Method 8260.

The initial soil sampling will involve the advancement of twelve (12) shallow soil borings within and in close proximity to the former drycleaner facility. Soil borings (10 total) will be advanced with a stainless steel hand auger and soil samples will be collected from 0-0.5’, 0.5- 2.0’, and 2’ intervals until refusal or the water table. All soil samples will be submitted to a fixed laboratory for analysis for the presence of volatile organics (VOCs).

Following soil sampling, piezometers will be installed at eight (8) locations.

The 6 shallow wells will be installed to approximate depths of 15 ft bls. One of the shallow wells will be installed within the facility using hand operated equipment. The wells will be screened to straddle the water table. The deep well is proposed for a total depth of 50 feet and will be screened between 45 and 50 feet, depending on site lithology. Well locations and construction details will be provided to the Department for approval prior to installation. Following installation, the wells will be surveyed and sampled in accordance with FDEP SOPs. All of the groundwater samples will be submitted to a fixed laboratory for VOC analysis by EPA Method 8260.

Subtask 3: Surveying/IDW Disposal Following all field activities, a surveyor will be subcontracted to develop a site plan and survey elevations of all sample locations. Investigative Derived Waste (IDW) will be disposed of at an appropriate off-site facility. GHD will coordinate manifest generation and IDW pickup.

Subtask 4: Site Assessment Report Upon receipt of analytical data, a Site Assessment Report (SAR) will be prepared. The SAR will include a summary of field methodology, field screening data, and laboratory analytical results. The SAR will also include conclusions and recommendations. The recommendations may include the use of active remediation to mitigate identified impacts.

GHD SERVICES INC

REQUEST FOR DRILLING BIDS

Courtesy Cleaners 700 S Osprey Ave, Sarasota, Sarasota County, Florida

FDEP Drycleaner Program Site

GHD Services Inc. (GHD) is requesting bids for continuous soil sampling to depth, groundwater profiling, and the construction of monitoring wells at the above-referenced project Site. The drycleaner is standalone facility with multiple buildings. Underground utility services are assumed to be present and active.

Expected lithology beneath the site includes gray to light gray fine sand from land surface to 3 feet below land surface (bls). Black to dark brown organic stained fine sand is expected at 3 feet bls, which is underlain by light yellowish brown fine sand to a depth of 12 feet bls. White calcareous fine sand is expected between 12 and 14 feet bls. Although not documented in the file, dense clay is expected to be encountered within 25 feet of land surface. The depth to water at the site is expected to be on the order of 3 to 5 feet bls.

The Driller will provide cost estimates to obtain continuous soil samples using direct-push techniques at one (1) location, advance seven (7) direct-push profiling points, install five (5) shallow monitoring wells, and one (1) deep well. GHD will obtain permission from the Owner and contact the statewide utility location services. The Driller will obtain all necessary well permits.

The following are the specifications:

Lithology Boring – one (1) Exterior Locations

1. The Driller will obtain continuous soil samples from surface to the total depth (50 feet) of investigation.

2. GHD desires to obtain representative soil samples from land surface to a projected total depth of 50 ft bls using hand auger and direct-push equipment.

3. Upon completion, the borehole shall be backfilled with Portland cement, or a Portland cement/bentonite mixture. The backfill material will be placed using a grout pump from the bottom up.

Direct-Push Sampling – seven (7) Exterior Locations

1. The Driller will obtain continuous soil samples from surface to the water table at all boring locations.

2. GHD desires to obtain representative soil (above the water table) and groundwater samples to a projected total depth of 50 ft bls at each location. Continuous soil samples will be collected from above the water table (estimated at 4 to 6 ft bls) and groundwater samples will be collected at approximately 10-ft intervals to the total depth, starting at the water table.

3. The boreholes shall be backfilled with Portland cement, or a Portland cement/bentonite mixture. The backfill material will be placed using a grout pump from the bottom up.

Exterior Shallow Monitoring Wells (5) Locations

1. During same mobilization, use direct-push techniques to install five exterior permanent monitoring wells. Driller shall specify the actual drilling method proposed. The actual depth will be determined in the field by GHD’s field representative based on the Site lithology and groundwater screening results. The projected total depths are 15 ft bls.

2. The wells will be constructed of 1-inch diameter, Schedule 40 PVC casing and 0.010-inch slotted, factory “pre-packed” screen.

3. The screened interval will be 10 feet in length, based on a field determination by GHD.

4. The remaining space between borehole wall and the well screen will be backfilled with 20/30 gradational silica sand up to approximately one foot above the well screen. A one foot section of fine (30/65 gradational silica) sand will be placed above the sand pack as a seal.

5. The sand pack and seal will be poured or tremied in place as the drive casing is removed. The

Driller will ensure proper placement of sand and verify that the sand pack has not bridged. The depth to the top of the sand pack will be verified.

6. The annular area above the fine sand seal will be topped with neat cement grout to prevent surface runoff from entering the annular area.

7. The wells will be completed at ground surface with a flush-mounted concrete pad, bolt-down steel cover and water-tight locking cap.

Exterior Deep Monitoring Well (1) Location

1. During same mobilization, use direct-push techniques to install one deep exterior permanent monitoring well. Driller shall specify the actual drilling method proposed. The actual depth groundwater screening results. The projected total depth is 50 ft bls.

2. The well will be constructed of 1-inch diameter, Schedule 40 PVC casing and 0.010-inch

3. The screened interval will be 5-10 feet in length, based on a field determination by GHD.

gradational silica sand up to approximately one foot above the well screen. A one foot section of fine (30/65 gradational silica) sand will be placed above the sand pack as a seal.

7. The well will be completed at ground surface with a flush-mounted concrete pad, bolt-down

All down-hole tools will be steam-cleaned between locations.

Following installation, each well will be developed by overpumping until the discharge stream is free of visible sediments. GHD will oversee the development process and determine when the development is complete at each location.

All discharge water and drill cuttings will be containerized in 55-gallon drums suitable for later shipping. The Driller shall provide the cost per drum in the lump sum bids below.

If additional mobilizations are required, the bid rates provided will be used.

Re: Non-Hazardous Drum Transport and Disposal – Courtesy Cleaners- Sarasota, FL

Mr. Moore:

The price for transport and disposal of 55-gallon drums of non-hazardous soil, mud, and/or water located in Sarasota, FL will be $110.00 per drum.

Please ensure that all drum lids are properly secured and that drums are on a level surface like the surrounding area (i.e., not on a pallet, etc.). A single False Mobilization fee will apply if our driver is not able to locate a drum on site or if we are unable to access the property due to a locked gate, etc.

Please feel free to contact me if you have any questions.

Respectfully, Robert Donado Erwin Remediation, Inc.
