Contract Summary
This document package contains Task Assignment Change Orders (TACOs) and related correspondence between the State of Florida Department of Environmental Protection (FDEP) and Environmental Consulting and Technology, Inc. (ECT) for environmental services at Crossroads Square Shopping Center (ERIC_4625) in Jacksonville, Florida. The indefinite delivery contract is identified as HW682, with the primary task assignment number DC006B managed by FDEP Project Manager Karlee Fowler and ECT Contract Manager Mark A. Culbreth.
The contract authorizes ECT to perform annual natural attenuation monitoring (NAM) and investigation-derived waste (IDW) management services at the shopping center site. TACO #2, effective July 26, 2024, reduced the compensation from $6,845.07 to $6,801.07 (a $44 reduction) due to decreased IDW costs resulting from a stolen drum that was to be transported and disposed. The total task value is $6,801.07 for 37 labor hours with completion scheduled for September 13, 2024. Deliverables include an Annual NAM Report due July 26, 2024 ($5,979.53), and IDW Manifest/Project Management due August 16, 2024 ($821.54). The services encompass groundwater sampling from seven monitoring wells, laboratory analysis using EPA Method 8260 for volatile organic halogens, and coordination of IDW disposal through subcontractors including Erwin Remediation ($85.00), with all other terms and conditions of the original task assignment remaining in effect.

Re: Crossroads Square Shopping Center (ERIC_4625) 8102 Blanding Boulevard Jacksonville, Florida 32244 Task Assignment DC006B Subtask 3 Deliverable
Dear Ms. Fowler:
Environmental Consulting & Technology, Inc. (ECT) was issued Task Assignment DC006B on September 14, 2023, to perform annual natural attenuation monitoring (NAM) at the Crossroads Square Shopping Center. Annual NAM was performed on February 14, 2024, which consisted of collecting groundwater samples from seven monitoring wells for laboratory analysis. Purge water generated during NAM was containerized in one Florida Department of Transportation-approved 55-gallon drum which was sampled and temporarily left onsite as shown in the photograph below.
One sample of the investigation derived waste (IDW) was collected (ID: IDW-20240214) and analyzed for volatile organic halogens (VOHs) using U.S. Environmental Protection Agency Method 8260D.
Ms. Kelly Crain, FDEP Crossroads Square Shopping Plaza (ERIC_4625) April 22, 2024 ectinc.com
Results of the analysis document that the IDW was nonhazardous. The laboratory analytical report is included as Attachment 1.
On April 22, 2024, Erwin Remediation, Inc. (Erwin) arrived onsite to remove the drum for proper disposal and could not locate the drum. The photograph below provided by Erwin shows that the drum was not at its original location when they arrived onsite. The drum is presumed to have been stolen from the site.
Should you have any questions regarding this submittal, please contact the undersigned.
Sincerely, ENVIRONMENTAL CONSULTING & TECHNOLOGY, INC.
