Monitoring September 22,2022

ECT will conduct groundwater sampling at the Crossroads Square Shopping Center site located in Jacksonville Florida. All work will be conducted in accordance with the scope of work and cost proposal submitted on August 9, 2022.

Subtask 1: Project Management Under this subtask is a percentage of the overall project and has been negotiated as a contract rate.

The wells to be sampled include monitoring wells MW001, MW002, MW003, MW004, MW005, MW006, and IW001. All groundwater samples will be analyzed for the volatile organic halogens by EPA Method 8260.

Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Subcontractors for IDW disposal were chosen on a lowest-cost basis. The Florida Department of Environmental Protection (FDEP) has a current and valid site access agreement for this site, signed August 29, 2016.

Documentation of the costs is included in the attached CEF, Level-of-Effort sheets, and the ECT proposal. The final deliverable due date is October 30, 2022 , and the completion date for the Task Assignment is December 30, 2022. The final cost total for DC006A is $ 6,477.81

Re: Non-Hazardous Drum Disposal Services 8102 Blanding Blvd, Jacksonville, FL & 7551 103rd St, Jacksonville, FL Lewis Proposal No. 220468LA

Dear Dr. Layton:

Lewis Environmental (Lewis) is pleased to present this proposal to Environmental Consulting & Technology, Inc. (ECT) for waste transportation and disposal services for 2 drums of IDW purge water. The following is offered for your review and consideration.

Scope of Work:

Lewis will provide a transport vehicle, CDL driver, drum dolly, and miscellaneous tools to complete the tasks as outlined below:

Lewis will provide a waste profile for approval and signature by the generator.

Lewis will arrive on-site and use a drum dolly to load the drum into a rack truck. The drum will be loaded into rack truck, secured, and hauled off site for disposal.

Lewis will supply a shipping document to generator for signature prior to transportation of waste off-site.

The waste will be taken to an approved disposal facility.

Lewis will provide signed waste manifests and disposal documentation to ETC.

Pricing:

Drum Waste Transportation and Disposal Waste Transportation (est. 1) $1,430.00/Load Non-Hazardous Drum Disposal (est. 2) $140.00/Drum

Estimated Project Total $1,710.00

Assumptions and Clarifications:

Lewis assumes a ETC site representative will be onsite to assist with site access and locating the drums if needed.

Lewis assumes free and clear access to the site on scheduled workdays.

Waste disposal pricing is contingent upon facility approval.

Lewis assumes drums are stored on a concrete or asphalt surface and are accessible by a drum dolly. If access is limiting, then additional charges may apply.

All waste must meet acceptability criteria of the designated disposal facility and comply with Local, State, and Federal regulations. This proposal is contingent upon sample evaluation and waste approval at the disposal facility. Any waste not meeting facility criteria will be subject to surcharges or possible facility rejection. In the event of a waste surcharge or facility rejection the associated costs will be invoiced additionally.

Lewis assumes the generator will have a representative on site to sign shipping/waste tracking documents.

Lewis assumes all drums to be in DOT shippable condition and have secure lids, be of solid constitution and not damaged or leaking. Drums not in DOT shippable condition will require overpacking at $350.00 per overpack drum.

Lewis assumes any analytical required by the waste facility will be provided by ECT.

Lewis assumes the waste stream to be disposed of is non-hazardous IDW waste with no other components. It is assumed there are no other constituents or chemical characteristics that would qualify this waste as RCRA hazardous. If any additional components (hazardous or non-hazardous) are mixed into this product, then additional costs may apply.

All minimum waste volumes/weights assessed by the disposal facility will be passed on to the generator/client.

The disposal pricing provided represents one cost effective means for proper disposal of waste(s) associated with this proposal. Ultimately the generator of the waste is responsible for the selection of the disposal, recycling or treatment facility.

We appreciate the opportunity to offer Environmental Consulting & Technology, Inc.

pricing for this project and look forward to working with you its successful completion. If you have any questions, please do not hesitate to contact me at 484-942-3093. Thank you for your time and consideration.

Sincerely, Rachael O’Connor Environmental Compliance Specialist

Re: Proposal for Crossroads Square Shopping Center

Annual Monitoring of Natural Attenuation

8102 Blanding Boulevard

Jacksonville, Florida

ERIC_4625

Contract No. HW682

Dear Ms. Walsh:

Environmental Consulting & Technology, Inc. (ECT) has prepared this scope of work to perform the annual natural attenuation monitoring and to prepare a Natural Attenuation Monitoring Report for the Crossroads Square Shopping Center in Jacksonville, Florida.

Scope of Work

ECT proposes to conduct the tasks necessary to complete the natural attenuation monitoring, consisting of the collection and analysis of groundwater samples from 7 monitoring wells and dispose of the investigative derived wastes. The wells to be sampled include monitoring wells MW001, MW002, MW003, MW004, MW005, MW006, and IW001. All groundwater samples will be analyzed for the volatile organic halogens by EPA Method 8260. The scope of work is detailed in the attached Level of Effort Worksheet.

Because this site is new to ECT, we are requesting additional time to generate base maps and other related figures as well as to recreate tables. In addition, ECT is requesting time to prepare a Health and Safety Plan for this site. Future monitoring activities will rely on this Health and Safety Plan so it is anticipated that this plan will need to be prepared only once.

Upon receipt of the laboratory data, ECT will provide the Florida Department of Environmental

Protection (FDEP) copies of the field notes, sample forms, laboratory report, and updated figures and tables. The Natural Attenuation Monitoring Report will subsequently be prepared and provided to the FDEP Project Manager.

Deliverable

ECT will submit the Natural Attenuation Monitoring Report on October 30, 2022 and close the Task

Order on December 30, 2022.

FDEP

ectinc.com

Cost

ECT proposes to conduct this scope of work for a cost of $6,570.69 in accordance with the Level of

Effort and Cost Estimate Form spreadsheets attached to this proposal.

There are three noteworthy factors that affect the cost for this project: IDW disposal, mobilization to and from the site, and per diem allowance.

IDW Disposal

This cost includes disposal of one drum of Investigative Derived Waste consisting of purge water from the seven wells. Quotes were obtained for disposal of the IDW. These quotes are summarized below and are provided as an attachment to this proposal.

Subcontractor Cost

Clark Environmental $176.00

Crystal Clean $292.92

Lewis Environmental $1,710.00

Based on these quotes, Clark Environmental will be subcontracted to dispose of the IDW.

Mobilization

The distance from ECT’s Orlando office to the Crossroads Square Shopping Center is 140 miles according to Google Maps. The travel time between ECT’s Orlando office and the site is estimated to be 2.5 hours, depending on traffic conditions in Orlando and Jacksonville.

Per Diem Allowance

Field activities related to measuring water levels and sampling the seven monitoring wells, plus managing the IDW is estimated to be approximately 9.5 hours. The travel time to and from the site, plus the time on-site is estimated to be 14.5 hours. Due to the time required to mobilize to the site from ECT’s Orlando office, conduct the sampling, and return to Orlando, ECT is requesting one night in a hotel and a per diem allowance.

Actual spreadsheets are being provided concurrently with this proposal. Subcontractor quotes are also provided with this proposal.
