HW682 DC015A

This Task Assignment authorizes ECT to conduct groundwater sampling activities in accordance with the May 11, 2023, Scope of Work and Cost Estimate.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Groundwater Sampling/MAGS This subtask includes labor, travel, analytical and equipment costs associated with the groundwater sampling activities at the site. Monitor wells MW001, MW004 and MW007 will be sampled and analyzed for VOHs via EPA Method 8260. Additionally, to evaluate whether there is a residual mass of chlorinated solvents in the vadose zone, a MAGS test will be performed according to the Scope of Work attached. The groundwater and MAGS analytical data, as well as recommendations on a path forward, will be presented in the Confirmation Sampling Report

Subtask 3: IDW This subtask includes labor and subcontractor costs associated with IDW transportation and disposal.

A site access agreement has been executed for this site and is located in the site file.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $11,314.60 with 71 labor hours.

Re: Proposal for Loretto Cleaners

Groundwater Monitoring Report

11701-1 San Jose Boulevard

Jacksonville, Florida

ERIC_4269

Contract No. HW682

Dear Ms. Shirah:

On April 14, 2023 Environmental Consulting & Technology, Inc. (ECT) submitted to the Florida

Department of Environmental Protection (FDEP), the Annual Natural Attenuation Monitoring (NAM)

Report for Loretto Cleaners. The results documented that only one sample, i.e. the sample collected from monitoring well MW007, contained a detectable concentration of solvents. However, at 2.9 ug/L, the concentration detected did not exceed its groundwater cleanup target level (GCTL). None of the other chlorinated solvents were detected in the sample collected from monitoring well MW007, nor did any of the other samples contain detectable concentrations of the chlorinated solvents.

Given the recent and historical groundwater data, ECT recommended conducting another round of groundwater sampling to confirm that groundwater quality meets the No Further Action criteria for closure under Risk Management Option I in Chapter 62-780.680(1). In addition, ECT proposed conducting a Modified Air Gas Sampling (MAGS) test to evaluate whether residual solvents may exist in the vadose zone at the site.

ECT has prepared this scope of work to perform the confirmation groundwater sampling and MAGS test. Should these data confirm that the groundwater quality data meet the no further action criteria, and the MAGs test results indicate that there is not a residual mass of chlorinated solvents in the vadose zone, ECT will prepare a Confirmation Sampling Report including a No Further Action Proposal recommending closure under RMO I. Should the data not support closure under RMO I, the

Confirmation Sampling Report will recommend continued NAM.

SubTask 1 Project Management

This Sub Task consists of calculated project management labor, based upon 13% of the direct labor costs in this proposal, and includes fringe, overhead, other direct costs (ODCs) and completion fees.

Ms. Ginger Shirah FDEP

Loretto Cleaners ectinc.com

SubTask 2 Scope of Work

Based on the recommendation in the April 14, 2023, Natural Attenuation Monitoring Report, ECT proposes to conduct the following tasks to evaluate whether the site meets the No Further Action criteria under RMO I.

ECT proposes collecting depth-to-water (DTW) measurements at monitoring wells MW001, MW002, MW004, MW005, MW007, and MW008 and collecting groundwater samples from monitoring wells

MW001, MW004 and MW007. All groundwater samples will be analyzed by United States

Environmental Protection Agency (USEPA) Method 8260 for the volatile organic halogens (VOHs). The scope of work is detailed in the attached Level of Effort Worksheet.

Additionally, to evaluate whether there is a residual mass of chlorinated solvents in the vadose zone a MAGS test will be performed. A comment letter dated October 10, 2014, indicated that 7 MAGS wells were utilized during the initial MAGS survey. The comment letter also provided some the MAGS operating parameter that will be used in the proposed MAGS Test which were as follows:

Applied vacuum range of 20 to 49 inches of water column.

Air flow rate range of 14 to 40 standard cubic feet per minute.

Operation at MAGS wells varied from 30 to 60 minutes. ECT used 60 minutes for each well in this proposal. The field parameter will be utilized to determine whether less than 60 minutes will be required.

Since documentation on the FDEP database Oculus for the Site was limited, the locations of the MAGS wells are not known. ECT proposes that during the groundwater sampling event, time be included to locate the MAGS wells and verify that they are viable for testing. It was documented that four MAGS wells were located inside the facility and three MAGS wells were in the parking lot, southeast of the facility.

The groundwater and MAGS analytical data will be presented in the Confirmation Sampling Report and include recommendations for further actions based on the new data. A summary of SubTask 2 activities is included in Table 1.

The scope of work for SubTask 3 consists of transportation and disposal (T&D) disposal of IDW. ECT proposes that the waste manifest be submitted as the SubTask 3 deliverable. Quotes for T&D of the
