Offerors must complete a detailed table identifying committed small business subcontractors, specifying their business categories (such as Small Disadvantaged Business, Woman-Owned Small Business, HUBZone, Veteran-Owned Small Business), proposed dollar values, and percentage of total contract value. Both large and small businesses must complete this form, which will be evaluated as part of Factor 3 - Small Business Utilization and Participation in the solicitation.
