About this file
This is a Past Performance evaluation factor document for a fire alarm service contract solicitation (36C25626Q0430) issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 16. Offerors must demonstrate experience as prime contractors managing fire alarm service contracts in hospital or clinical environments, with projects that are either 100% completed or ongoing and more than 50% complete.
Offerors must submit detailed project information including contract numbers, physical locations, points of contact for the Contracting Officer Representative or Contracting Officer, scope of work descriptions, identified subcontractors, project timelines, and contract award amounts. Evaluation will focus on recent and relevant projects comparable in size, scope, and complexity to assess the offeror's demonstrated ability to deliver quality results on time, within budget, and in compliance with applicable regulations. Offerors must complete a Performance Relevancy Survey (Exhibit A) and coordinate with prior clients to submit a Past and Present Performance Questionnaire (Exhibit B) to the Contracting Office, with completed questionnaires ideally no more than one year old. Offerors with limited or no past performance may receive ratings based on subcontractor capabilities or key personnel qualifications if an executed Subcontractor Information and Consent Form (Exhibit C) is included in the proposal. Offerors without relevant past performance history will receive a neutral rating; however, such proposals may be considered unsuccessful when compared to proposals with demonstrated relevant experience.

An offeror with limited or no past performance may receive a rating based on the evaluation of its subcontractors and/or key personnel, provided an EXHIBIT C SUBCONTRACTOR INFORMATION AND CONSENT FORM has been executed and included in the proposal for proposed subcontractors and/or potential key personnel not employed by the offeror. These ratings shall have the same weight as the ratings of the offeror. Offerors without a record of relevant past performance will be assigned a neutral rating in accordance with FAR 15.305(a)(2)(iv). However, the proposal of an offeror with no relevant Past Performance history, while rated “Neutral” in Past Performance, may not represent the most advantageous proposal to the Government and thus, may be an unsuccessful proposal when compared to the proposal of other offerors.
Evaluation Methodology: Past performance will be evaluated based on the projects provided by the offeror, and performance information from government databases. Focus will be on recent and relevant contracts that are comparable in size, scope, and complexity. The Intent of this evaluation is to evaluate the demonstrated ability to successfully complete comparable requirements, ensuring they have the experience and capability to deliver quality results on time, within budget, and incompliance with guidelines and regulations.
