About this file
This is a Fire/Life Safety System Inspection, Testing and/or Maintenance Certificate for Fayetteville VAMC Building 3 located at 1100 N College Ave, Fayetteville, AR 72703. The inspection was completed on September 8, 2025, by Veterans Fire and Life Safety, with inspector Mike Gill conducting the work. The inspection and testing were performed in compliance with The Joint Commission/VHA Standards and Joint Commission/VHA Clarification Memorandum on EC & LS Standards.
The inspection covered 74 total devices across multiple categories. All devices passed testing except for four sealed lead acid backup batteries (18 Ah and 26 Ah rated capacity), which failed load testing and require replacement per NFPA 72 Table 14.4.3.2 (9)(5). Successfully tested devices include 1 duct detector, 4 heat detectors, 17 pull stations, 14 smoke detectors, 2 speakers, 22 speaker/strobes, 8strobes, 1 control panel, and 1 power supply. Auxiliary functions testing confirmed proper operation of AHU shutdown, shunt trips for elevators, and elevator recall functions. All fire alarm system components were installed on August 7, 2024, and are within their warranty period of one to two years in service. The system control unit is located at the 1st Main FACP with breaker protection. The proposed solution report identifies battery replacement as the only required remedial action, available on a Time and Materials basis.
