About this file

This is a Fire/Life Safety System Inspection, Testing and/or Maintenance Certificate documenting an annual inspection performed on September 15, 2025, at Fayetteville VAMC Leroy Pond (1100 N College Ave, Fayetteville, AR 72703). Veterans Fire and Life Safety conducted the inspection in compliance with The Joint Commission/VHA Standards and latest Joint Commission/VHA Clarification Memorandum on EC & LS Standards. The inspection was performed by Rod Davis (Manager, Veterans Fire & Life Safety, St Peters, MO 63376) with building contact Travis Ryan (Safety Specialist, 479-225-6412). The system control unit is a Notifier manufacturer unit installed on August 6, 2024, located in the 1st North Electrical Room with breaker current protection.

The inspection tested a total of 190 devices across multiple categories, all of which passed testing. Testing included 2 tamper switches (semi-annual), 1 waterflow switch (semi-annual), 6 duct detectors, 1 heat detector, 20 pull stations, 34 smoke detectors, 82 horn/strobes, 10strobes, 5 monitor devices, 2 monitoring devices, 6 initiating devices, 3 annunciators, 10 batteries, 1 control panel, 3 power supplies, and 4 programmable relays. All devices were tested in accordance with NFPA 72 (2022) and NFPA 90A (2024) standards. Duct detector batteries were replaced during the inspection. All batteries tested maintained adequate voltage levels (ranging from 12.30 to 13.38 volts for sealed lead acid units). No failed devices were identified, and auxiliary functions including AHU shutdowns and programmable relays all passed testing.
