About this file

This is a Fire/Life Safety System Inspection, Testing and/or Maintenance Certificate for Fayetteville VAMC Building 8, completed by Veterans Fire and Life Safety on September 15, 2025. The inspection was performed in compliance with The Joint Commission/VHA Standards and the latest Joint Commission/VHA Clarification Memorandum on EC & LS Standards. Inspector Rod Davis conducted the annual inspection, with building contact Travis Ryan at 479-225-6412. The inspection covered 42 total devices across multiple categories including tamper switches, waterflow switches, pull stations, smoke detectors, speakers, strobes, speaker/strobes, batteries, power supplies, and programmable relays.

The inspection results identified two failed batteries (ScanID 7367563 and 89471371, both located at Ground level) that require replacement due to failed load tests, per NFPA 72 Table 14.4.3.2 (9) standards. All other devices tested passed their inspections. The passed devices included 3 tamper switches (annual test), 1 waterflow switch (semi-annual test), 7 pull stations, 1 smoke detector, 9 speakers, 3 speaker/strobes, 6 strobes, 4 passing batteries, 1 power supply, and 5 programmable relays. The Proposed Solutions Report indicates both failed batteries are available for replacement on a Time and Materials (T/M) basis with no specific model number or estimated cost provided. Testing was noted as performed by others for supervisory devices. The most recently installed devices date to August 6, 2024, while some supervisory devices (tamper switches and waterflow switch) have been in service for 25 years or longer.
