About this file
This is a Fire/Life Safety System Inspection, Testing and/or Maintenance Certificate for Fayetteville VAMC Building 6, completed on September 1, 2025, by Vets Fire & Life Safety. The inspection was performed in compliance with The Joint Commission/VHA Standards and the latest Joint Commission/VHA Clarification Memorandum on EC & LS Standards, with inspector Mike Gill conducting the evaluation at the facility located at 1100 N College Ave, Fayetteville, AR 72703, and Travis Ryan serving as the building contact.
The inspection examined 35 total fire alarm and life safety devices across multiple categories. Two backup batteries located in Basement 6B Storage failed load testing and require replacement on a time and materials basis, violating NFPA 72 Table 14.4.3.2 (9) standards for voltage and internal ohmic values. Of the devices tested in Q3 2025, two batteries failed while all other tested components passed, including eight smoke detectors, four pull stations, two heat detectors, ten horn/strobes, and eight strobes. Thirty-three devices remain untested year-to-date, including the power supply and various notification appliances throughout the basement, first, and second floors. All devices documented in the inventory were installed between August 7-10, 2024, falling within the one to two-year service window, and no manufacturer recalls or replacement programs were identified during the inspection.
