1. PROJECT LOCATION / GRAVEL EXTRACTION SITES:

1.1 All work areas under this contract will be performed within the geographical boundaries of Joint Base Elmendorf – Richardson (JBER), Alaska. Work areas will be identified on the issued delivery orders.

1.2 Gravel Extraction Sites

1.2.1 JBER Gravel Extraction Area

: The gravel extraction site (see JBER site map) is available for this contract for the contractor’s use for obtaining the aggregate needed for the asphalt pavement, JBER course, sub-JBER, classified fill/backfill, pit run, etc. for both JBER course, and sub-JBER issued delivery orders.

1.2.2 JBER Gravel Extraction Area:

The gravel extraction site (see JBER site map) is available for this contract for the contractor’s use for obtaining the aggregate needed for the asphalt pavement, JBER course, sub-JBER, classified fill/backfill, pit run, etc. for both JBER and sub-JBER issued delivery orders. The gravel extraction site is located off the Davis Highway on JBER Richardson.

1.2.3 The JBER gravel extraction sites

are available for the contractor’s use for the processing of topsoil, concrete, or like materials as part of this contract. All or portions of this type of material will not be available at this site. The contractor will be required to import any materials not available at this site.

1.2.4 The contractor is responsible for performing the necessary testing to determine the usability and reject level of aggregate from the gravel extraction sites. The government makes no claims that the aggregate from the gravel extraction sites will meet all the aggregate criteria in the specifications.

1.2.5 The use of off-JBER aggregate sources and off-JBER asphalt plants, when at the discretion of the contractor, shall not constitute a change in the unit price of on-JBER line items.

1.2.6 The contractor may be required to share the gravel extraction sites with other contractors.

The gravel extraction sites are owned by the government. The gravel extraction sites are managed by the 673rd

CES Pit Manager, Mr. Robert McElroy, and can be reached at 552-1601. He is responsible for determining the work area for each contractor utilizing all pits on JBER.

1.2.7 The contractor shall submit a gravel extraction site mining/equipment location/restoration plan to the contracting officer for approval for each gravel extraction site to be used by the contractor. See specification section 1020-para 9 for Borrow Pit procedures and requirements.

General Requirements

2. ISSUING OFFICE / NOTIFICATION OF WORK START:

2.1 For all issued delivery orders:

2.1.1 The following office is authorized to place delivery orders and will be responsible for the administration of placed delivery orders:

673rd Contracting Squadron/LGCA 10480 22 nd

2.1.2 The Contractor shall give at least 7 working days advance notice in writing to the Contracting Officer, unless approved otherwise, prior to starting any work under this contract and prior to starting on-site work on each delivery order. Provide an “on-site start work” notice to the Contracting Officer for establishing the “on-site” construction start and completion dates.

2.1.3 Coordinate with appropriate agencies, including JBER Communications Cable Maintenance (Item 7 through 17 of 673rd Wing Form 3). Notify JBER Communications Cable Maintenance at least 48 hours in advance, before actual construction/ excavation commences, to ensure proper on-site monitoring of red zone areas as defined in Eleventh Air Force Regulation 700-5. Red areas are defined in Eleventh Air Force Regulation 700-5, as a distance from communication lines within which excavation will be by hand-digging or other approved methods around the communication cables.

2.2 Building Custodian: The contractor shall notify the building custodian of the approved work schedule at 7 days and 3 days prior to starting paving work, unless approved otherwise by the project engineer/project inspector. In the housing areas, each housing unit and the housing manager shall be notified.

3. WORK SCHEDULE:

3.1 General Work Hours

: Construction work hours are limited to Monday through Friday from 0600 to 1900 hours, excluding federal holidays, unless approved otherwise in writing by the Contracting Officer. Work hours for Airfield work are indicated in specification section 1020-para 7.

3.1.1 The general work hours apply to work activities which require government inspection to insure compliance with the technical specifications. This does not include heavy equipment maintenance/repairs, plant mobilizations/demobilizations, crushing operations, driving to the job site or borrow pit, field surveying, field & lab testing, & the like. When performed, heavy equipment maintenance/ minor repairs shall be performed in the borrow pit unless approved otherwise.

3.1.2 Housing & Dorm Work Hours

: Construction work hours in Military Family Housing areas and Dorm areas are limited to Monday through Friday from 0700 thru 1900 hours, excluding federal holidays, unless approved otherwise in writing by the contracting officer.

3.1.3 Should the Contractor desire to work other than these hours, the Contractor must request approval of deviation in writing at least 24 hours prior to the period of the intended deviation, but not later than Friday 0900 hours for working on the weekend. The Contractor shall not accomplish the deviation in work hours unless approved by the Contracting Officer.

4. FIRE, SAFETY, AND OTHER REGULATIONS:

4.1 The Contractor shall comply with all national, state, local and Air Force fire and safety regulations. A copy of Air Force fire and safety regulations can be reviewed at the Contract Management Office, 673rd Civil Engineering Squadron, 6326 Arctic Warrior Dr.

4.2 Prior to using any open flame devices, welding equipment and other similar operations contractor and subcontractor personnel performing this type of work shall be certified to issue AF Form 592, USAF Welding, Cutting and Brazing Permit. Certification training is provided by appointment and can be scheduled by contacting Technical Services at 552-2620/3267. Compliance with the requirements are in NFPA Standard No 241 (Bldg Construction and Demolition Operations, NFPA 51B, AFOSH 91-5 and OSHA 1910-252 is mandatory.

4.3 Training of Contractor Employees: Contractor shall ensure that all their employees, including the employees of subcontractors receive an initial Fire Prevention Orientation briefing prior to beginning any work under the scope of this contract. This briefing is given by the JBER Fire Department on every Thursday each month at 0800 at 5126 Arctic Warrior Dr., Fire Station #2 and shall be scheduled through written request to the Fire Department at least 72 hours prior to the intended date of the briefing. The Contractor shall submit a written record to the Contracting Officer indicating that all of his/her employees and subcontractor employees have received this briefing. As new employees of the Contractor and subcontractor are added during the performance of this contract, update the written record to the Contracting Officer. The Contracting Officer shall retain the right to spot check employees to verify if they have been provided the briefing. As new employees of the Contractor and subcontractor are added during the performance of this contract, the Contractor shall submit a written record of all of his/her employees and subcontractor employees receiving this briefing. The Contracting Officer shall retain the right to spot check employees to verify if they have been provided the briefing.

4.4 Contractor Fire Prevention Responsibility:

4.4.1

The contractor shall provide a “firewatch” person during all welding performed, which includes any welding work on heavy equipment anywhere on-JBER, as well as, in the borrow pits.

4.4.2 It is the inherent responsibility of the Contractor to practice good fire prevention measures while working on JBER.

4.4.2.1 Contractor personnel shall confine smoking to designated smoking area(s).

4.4.2.2 Flammable paints, oil, varnishes, etc., shall be stored in a metal storage cabinet or in outside storage in a controlled area. All containers shall be labeled correctly in accordance with all Federal, State, Local, and Air Force requirements. Flammables being used outside of these areas are limited to a one day supply.

4.4.2.3 Temporary wiring must be in compliance with Article 305 of the National Electric Code.

4.4.2.4 The Contractor will be responsible for notifying the JBER Fire Department and 773rd CES Alarm shop prior to any alarm or sprinkler system shutdowns at least 48 hours in advance.

4.4.2.5 Welding, cutting or brazing will only be done after a valid AF Form 592 has been issued by an individual certified by the JBER Fire Protection Flight to issue welding permits. Daily permits

1010-4 are required.

4.4.2.6 Fire extinguishers required during performance of this contract will be supplied by the Contractor.

4.4.3 Questions concerning fire prevention can be referred to the Technical Services Section at extension 552-2620/3267.

5. AS-BUILT DRAWINGS: Provide as required on each individual delivery order.

6. SAMPLING AND TESTING:

6.1 All tests shall be performed using specified testing procedures, or if no procedure is specified, the standard testing procedure used by the ASTM will be used. The Contractor shall verbally notify the Contracting Officer or his/her designated representatives at least 24 hours in advance when such testing will be performed so that the test may be observed by the Contracting Officer and/ or his/her designated representatives.

6.4 In addition to the specified testing requirements herein, the Government may request additional verification testing at the contractor’s expense. All necessary samples and/or work associated with such samples for such testing shall be provided by the Contractor at no additional cost to the Government. Repairs required to the work to correct the evidence of such Governmental testing, shall be replaced at the Contractor's expense within 24 hours unless otherwise directed in writing by the Contracting Officer.

7. UTILITIES:

7.1 Reasonable amounts of water, electricity and other available utilities will be made available to the Contractor at no cost to the Contractor. The Contractor shall provide the utility connections at no addition costs to the government. The Contractor shall exercise strict conservation practices in the use of these utilities. Should the Contractor be found not using utility conservation practices by the contracting officer or his authorized representative, then the cost of this utility use will be passed along to the Contractor for the remaining performance period of the contract.

7.2 Telephone services:

The contractor is responsible for securing telephone service for his use from the local off JBER telephone company at no cost to the government.

8. UTILITY OUTAGES:

8.1 Scheduled: Request for all outages and interruption of electricity or other utility services shall be made in writing through the contract inspector to the utility manager, copy furnished to the Contracting Office at least 7 working days (excluding Federal holidays and weekends) prior to the desired period of outage. All outages are subject to written approval by the Utility Manager and will be authorized only at the convenience of the Government. This notification shall be made at least 7 days prior to the utility outage with a follow-up notification made 24 hours prior to the utility outage in sufficient time to give the utility personnel ample opportunity to arrange and schedule discontinuance of utility services. The Contractor shall hold all outages to a minimum number and of the shortest duration possible and only as necessary to accomplish the required work.

8.2 Unscheduled: The Contractor shall reimburse the government for man-hours, materials, and circuit costs when damage to or interruption of service on any cable or utility occurs through negligence of the excavator/contractor, including any failure to obtain proper coordination of the Work Clearance Request prior to commencement of construction or excavation in accordance with paragraph 9 of this specification section.

9. WORK CLEARANCE REQUEST:

9.1 JBER - JBER

9.1.1 Permit: Before any excavation, alteration or other construction work is begun for JBER issued delivery orders, the Contractor shall obtain and fully process a 3rd (673rd) Wing Form 3, Civil Engineer Work Clearance Request. The form requires extensive coordination with several multiple agencies, including furnishing all locations, detailed information and sketches regarding any proposed excavation(s). The contractor shall process a separate request for each planned excavation site that has an approved delivery order. Some sites may require more than one permit because of size and/or scope. Since this form requires extensive coordination with several agencies, expect 5 working days for completion. This form can be obtained from the Construction Management Office, Civil Engineering, JBER, 6326 Arctic Warrior Dr; and it shall be prominently displayed on the job site at all times for review by the Contracting Officer and/or his/her designated representative.

9.1.2 Coordinate with appropriate utility locating agencies, both on-JBER and off-JBER, as shown on the Work Clearance Request 3rd (673rd

) Wing Form 3. Initially, this form will be given to the contractor during the pre-construction conference. Contractor shall complete and follow all instructions on this form before the start of work IAW Wing Instructions (WI 32-1007), “Safeguarding Utilities from Damage”. The contractor is considered the “construction agency” for compliance with WI 32-1007. A copy of WI 32-1007 will be available to the contractor during the pre-construction conference.

9.1.3 The Work Clearance Request is not valid until it is signed by the approving officer (block 21), who will not sign until all affected utility locates are marked in the field and all utility locating agency signatures are on this form. Excavation work must commence within 30 days after the approving officer’s signature, otherwise the work clearance request is no longer valid and a new work clearance request will be required. It is the contractors responsibility to maintain the locate markings until no longer needed to complete the work.

9.1.4 Contractor shall notify the project inspector immediately if new utilities were installed by others and not marked after all initial locates were marked.

9.1.5 Protection of Existing Utilities

: All underground utilities and overhead utilities with their support poles shall be safeguarded and protected from damage by the Contractor at all times, but especially when encountered or uncovered during excavation or backfilling operations.

9.1.5.1 Communications Cables

: Complete all excavations in red area(s), as defined by Eleventh Air Force Regulation 700-5, by hand rather than machine methods, or expose cables by hand-digging prior to completing excavation by machine.

9.1.5.1.1 Contractor shall restore damaged cable(s) and circuits to their original condition and be

1010-6 fully operational. If the contractor fails to fully process 3rd (673rd) Wing Form 3, or fails to comply with its instructions, or fails to follow 673rd WG/SCLPK (JBER Communications Cable Maintenance) instructions appended to 3rd (673rd

) Wing Form 3, the contractor shall be responsible for any and all damage that occur. Notification of damage shall be made immediately to the Telephone Trouble desk, 552-4647, and to the Contracting Officer; restoration efforts shall commence within 1 hour. Failure to commence restoration within the required time limit, or to make continuous progress toward restoration, or to complete acceptable restoration within a 4-hour time limit or an alternate time limit negotiated with and established by the Contracting Officer, shall result in suspension of contract work until acceptable restoration is completed and approved by the Contracting Officer.

9.1.5.1.2 In cases where a cable is damaged for reasons noted above, the Contractor shall, if lacking the qualified trades in his organization, employ as necessary such qualified trades or employ another Contractor having such trades to restore the cable and circuits to original condition and full operation at no expense to the Government.

9.1.5.1.3 The Government reserves the right to protect a damaged cable from further damage or deterioration and the right to restore priority circuits; these actions shall not relieve the Contractor of his full responsibility for restoration.

9.2 JBER - Richardson

9.2.1 Permit: Before any excavation, alteration or other construction work is begun on JBER Richardson issued delivery orders, the Contractor shall obtain and fully process a digging permit.

The contractor is responsible for obtaining the signatures from all of the responsible officials identified on the permit. The contractor shall provide a drawing showing the limits of work identified on the delivery order for markup to accompany the permit. An approved copy of the excavation permit shall be in the possession of the project superintendent. The contractor shall coordinate all on-site utilities marking with the responsible official. Permits shall be turned into the 773rd

CES Customer Service Office Richardson for final processing.

10. ACCESS TO CONSTRUCTION SITE / HAUL ROUTE:

10.1 The contractor will provide proposed access routes to the construction site, haul routes, and certain portions of existing parking areas for use by the contractor and his work force to the project engineer / project inspector for acceptance.

10.2 The contractor shall submit all proposed haul routes to the project inspector/project engineer for acceptance at least 7 calendar days prior to starting hauling, unless indicated otherwise.

Proposed haul routes shall include: to the construction site from the asphalt plant, from the borrow pit, from off-JBER, and to the off-JBER disposal site, etc, for each delivery order. Haul routes are subject to change by the government.

10.3 The contractor shall provide daily

clean-up of the approved haul routes that the contractor is responsible for dropping gravel or debris. When required by the government, the contractor shall provide a water truck for watering down the haul routes for dust control, etc.

10.4 The Security/Military Police will enforce traffic regulations, parking area assignments, and security. The Contractor shall cooperate and coordinate to the fullest extent. The Security/Military

Police are not responsible for the security of the contractor’s materials, equipment, etc..

10.4.1 All Contractors are advised that it is a violation of a JBER traffic regulation to exceed 10 miles per hour while passing military personnel in formation, or a military group running or walking along the side of the road. Driving privileges on this installation may be suspended or canceled for violating traffic regulations or applicable state laws and will not be considered an excusable delay under the contract. Each Contractor must ensure that all of its employees have been informed of this information.

10.5 Contractor shall provide to the Contracting Officer, security/military police, and entry control personnel, 72 hrs advance notice for each entry into restricted areas, unless approved otherwise by the Contracting Officer. Delays due to lack of immediately available military escort for Contractor personnel entering these areas shall be at Contractor's expense.

10.6 Free Zones may be established by the Security Police for the contractor to accommodate his work. The contractor must meet the following requirements:

10.6.1 Contractor shall contact the airfield manager (for restricted areas on the airfield only) and the security/military police. They will then establish the “free zone” for performing construction and provide assistance as necessary. In addition, sufficient restricted area signs will be provided to the contractor by the government for placing around the “free zone” area to prevent access to the restricted area by the construction workers. Contractor shall furnish/install standing self-supporting posts and red rope. For JBER contact 673rd

SFS/SFOS at 552-5121.

10.6.2 Contractor shall be responsible for erecting and maintaining all the free zone boundaries.

(Temporary barriers may consist of rope, wire or fence.) Free zones and restricted area signs will be posted around the boundary on all avenues of approach at 100 foot intervals.

10.6.3 The contractor shall adhere to prescribed travel routes and use only the authorized entry control points depicted on the map.

10.6.4 When changes occur, the contractor shall provide an updated list of names, vehicle license numbers and working hours of personnel involved in the contract to the contracting officer and the security/military police.

10.6.5 For JBER, the contractor shall close the free zone and secure the restricted area after normal work hours. The contractor must contact the free zone monitor at 552-1817 at the end of each work day who will in turn notify CSC, 552-3105. CSC will dispatch a patrol to remove free zone signs and purge the area with the contractor and the free zone monitor.

10.6.6 Contractor shall coordinate all changes to the free zone before they take place with the security/military police. For JBER - JBER contact 673rd

SPS/SPOS at 552-5121.

10.7 Contractor shall be responsible for maintaining, repairing, and restoring existing parking areas, which are used by his work forces, to their original condition immediately prior to his completion of work under this contract. Normal wear and tear will be taken into account.

11. CONTRACTOR'S STORAGE AREA/OFFICE AREA:

11.1 Contractor’s Office Area for Airfield Work: The contracting officer will designate an area in the vicinity of the airfield for the contractor’s office for work on airfield delivery orders. The JBER gravel extraction areas are also available.

11.2 Contractor’s Office Area for Road Work: The contracting officer will designate an area in the vicinity of the road work for the contractor's office for work on road delivery orders. The JBER gravel extraction areas are also available. At the contractor's discretion, the contractor may elect to use only one office area for airfield and road delivery orders.

11.3 Contractor’s Storage Area

: The contractor shall use the JBER gravel extraction areas for the contractor’s storage area for items that are part of this contract.

11.4 The contractor shall be required to maintain this area clean. The contractor is responsible for providing the security for the contractor’s office area and storage area. The government will not be responsible for the security of the storage area and of any items stored in this area. The contractor will return the area in the same condition or better as when he received it.

12. BARRICADES:

12.1 Construct and maintain barricades as required by the Safety Officer, the MUTCD (Manual of Uniform Traffic Control Devices) and OSHA sufficient to prevent injury to persons and damage to property.

12.2 Cover unattended trenches and holes.

12.3 Provide and maintain warning lights and signs as necessary to prevent damage or injury. Keep warning lights turned on from dusk to dawn.

13. COLD WEATHER OPERATIONS:

13.1 If any part of this contract is pursued during periods of cold or inclement weather, the Contractor shall take all appropriate precautions to protect facilities and work involved in the contract scope from damage by cold/or inclement weather.

14. SUBMITTALS:

14.1 The contractor shall submit all required Material Submittals in accordance with instructions contained herein and as specified on AF Form 66, Schedule of Material Submittals, to the Contracting Officer for approval. The government shall be allotted 15 working days to properly evaluate each material submittal & re-submittal. Contractor shall consider this time to evaluate submittals in his construction schedule.

14.1.1 “30 Days Prior to Starting Work on Order” means that the contractor shall provide the material submittals (for the line items on the issued orders only) to the government for approval at least 15 days prior to starting on-site construction work for that issued order. When the material submittal for that line item is approved, that material submittal is approved for the entire duration of that contract year and does not have to re-submit for each order with that line item. Should there be a change from the approved material submittal, then it shall be re-submitted for re-approval.

14.2 Rejection of submittals for technical violations of the drawings or specifications will not be

1010-9 grounds for an extension of the performance period. If re-submittals are required, all parts of the original submittal package will be resubmitted together with changes so that coordination may be maintained.

14.3 All submittals will be submitted in 3 ring binders with clearly marked tabs separating each item. Submittals will be consolidated by type (i.e. dirt work, electrical) and all submittals for each specification section will be submitted at the same time.

14.4 Any certificates required for demonstrating proof of compliance of materials with specification requirements shall be executed in 4 copies. Each certificate shall be signed by an official authorized to certify on behalf of the manufacturing company and shall contain the name and address of the Contractor, the contract name and location, and the quantity and date or dates of shipment or delivery to which the certificates apply. Copies of laboratory test reports submitted with certificates shall contain the name and address of the testing laboratory and the date or dates of the test to which the report applies. Certification shall not be construed as relieving the Contractor from furnishing satisfactory material, if, after tests are performed on selected samples, the material is found not to meet the specific requirements.

15. CODES:

15.1 References to standards, codes, specifications, recommendations and regulations refer to the latest edition/printing issued prior to the proposal receipt date.

15.2 These Construction Documents, and the joint and several phases of construction hereby contemplated are to be governed, at all times, by applicable provisions of the Federal Law(s), including but not limited to, the latest amendments of the following:

15.2.1 Williams-Steiger Occupational Safety and Health Act of 1970, Public Law 91-596.

15.2.1.1 By agreement between the US Department of Labor, Occupational Safety and Health Administration (OSHA) and State Department of Labor, Division of Occupational Safety and Health (DOSH), and HQ 11th Air Force, applicable OSHA regulations as amended by the State of Alaska are enforced for Contractors and Contractor personnel onl y by DOSH. At no time will the level of protection afforded the worker be less than the strictest of either the Federal or State regulations.

15.2.1.2 Code of Federal Regulations, Title 29, Parts 1910 (Occupational Safety and Health Standards) and 1926 (Safety and Health Regulations for Construction).

15.2.2 Federal Standard 313B, as it pertains to the procurement, identification, marking and control of potentially toxic or hazardous materials, and to the reporting of information concerning these materials.

15.2.3 International Building Code, International Mechanical Code and International Plumbing code.

15.2.4 National Electric code (NEC).

15.2.5 NFPA 101, Life Safety Code.

15.3 Applicable portions of standards listed in the Specifications that are not in conflict with Contract Documents or with the Federal Acquisition Regulations (FAR).

15.4 All Air Force, 11th Air Force, 673rd Wing, and JBER Regulations as pertain to construction, safety, fire, health and associated activities are part of this contract, which includes WINGI 31-203 JBER Traffic Regulations

16. APPROVING OFFICIAL:

16.1 The approving official and point of contact for this contract is the Contracting Officer. All direction and contract requirements will be determined by the Contracting Officer. The project engineer / project inspector will work with the contractor to provide the joint estimated quantities.

The project engineer will designate the work areas, and the performance time for each delivery order which will be issued by the Contracting Officer.

16.2 The “Contracting Officer” as used in this contract is defined as the “Contracting Officer or his/her designated representative”. The “project engineer / project inspector / project manager” as used in this contract is defined as the “government’s project engineer / project inspector / project manager”.

17. MONUMENT RESTORATION:

17.1 Carefully maintain all bench marks, monuments, and other survey reference points. If it is necessary to disturb or destroy a bench mark, monument, or other survey reference point, notify the 673rd Civil Engineer Squadron / Engineering Technician Unit at 552-2765 / 5661 before proceeding. Any bench mark, monument, or other survey reference point already disturbed or destroyed requires immediate notification by the contractor to the Engineering Technician Unit.

17.2 All disturbed or destroyed bench marks, monuments, and other survey reference points shall be restored to their original location as nearly as possible, or as otherwise directed by the Contracting Officer. The specific location shall be tied to monuments using JBER local grid coordinates. When a bench mark, monument, and other survey reference point is restored, the record of the survey shall indicate the deviation from the original location. Field notes shall be recorded in hard bound field books, which shall become the property of the government.

18. SALVAGE:

18.1 Equipment and materials indicated for reuse shall be approved by the Contracting Officer before re-installation. Contract amount includes cleaning, conditioning and installing materials indicated for reuse. The following equipment and materials are indicated for reuse: None

18.2 Any excess equipment and materials selected to remain the property of the Government shall be removed by the Contractor and delivered to the Property Disposal Officer, 12737 Vandenburg Ave. The following materials are to be delivered for Government disposal: None

19. DISPOSAL OF MATERIALS:

19.1 Solid Wastes: Contractors will be required to dispose of all solid wastes generated by on-JBER delivery orders at state permitted off-JBER sanitary landfill facilities.

19.2 Minimum requirements for off-JBER

solid waste disposal are as follows:

19.2.1 All wastes shall be disposed of in a State of Alaska permitted solid waste disposal facility, such as the municipal sanitary landfill located near Hiland Road and the New Glenn Highway.

19.2.1.1 Contractor is responsible for paying all dumping fees for JBER materials.

19.2.1.2 There are currently no dumping fees

for JBER Richardson materials taken to the Hiland Landfill provided the contractor has the proper passes issued by JBER Richardson.

19.2.1.3 Contractor is responsible for keeping JBER materials separate from JBER Richardson materials that are disposed of at the Hiland Landfill. JBER materials cannot be mixed with JBER Richardson materials to avoid the dumping fees.

19.2.2 Waste hauling from JBER to off-JBER

disposal locations shall meet or exceed all requirements and restrictions levied by the State of Alaska and the Municipality of Anchorage, including:

19.2.2.1 Meeting of any weight restrictions for any public roadways used.

19.2.2.2 Covering and securing all loads of waste other than earth materials to ensure that no materials are lost from each load during transit.

19.2.2.3 Prevention of overloading with earth materials such that load contents are prevented from escaping during transit.

19.3 Exceptions

to this policy are shown in specification section 1020-para10, “Disposal of JBER Materials”.

19.4 Liquid Wastes:

19.4.1 Runoff and residues generated from any process including water-washing processes shall not be directly wasted to the sanitary sewer or to the storm drain system. They shall be collected, sampled and disposed of based on the results of the analysis of samples, compliance with EPA requirements and this specification. Contractor shall obtain the necessary permits from the State of Alaska for the discharge of industrial waste water.

20. ENVIRONMENTAL CONCERNS & HEALTH HAZARDS:

20.1 The Contractor shall comply with the following environmental sections of this specification for JBER projects:

20.1.1 Environmental Protection/Hazardous Materials: The Contractor shall strictly comply with Section 1120 of this specification for protection of personnel, Government property, and the environment, and for hazardous material handling, use and disposal.

20.1.2 Hazardous Waste: The Contractor shall strictly comply with Section 1121 of this specification for hazardous waste accumulation and disposal.

20.2 note used

20.3 Flight line Hearing Protection

20.3.1 The contractor shall comply with Air Force Occupational Safety and Health Standard 48-19, Hazardous Noise Program.

20.3.2 When working on the flight line approved hearing protection is required. This includes approved ear plugs, such as E.A.R. foam plugs, V51R, triple flange, or other fitted plugs. This includes approved ear muffs such as Willson, Peltor, Bilsom, David Clark, etc.

21. DD FORM 1354: Upon completion and acceptance of a JBER delivery order, the contractor shall complete a DD Form 1354, “Transfer and Acceptance of Military Real Property”, prepared according to Air Force Regulations. Form shall be obtained from the Contract Management office.

Not every delivery order will require a DD Form 1354. Contact the project manager for guidance on which delivery order will require a DD Form 1354.

22. CONTRACTOR JBER ACCESS

22.1 All contractor vehicles require an AF Form 75 vehicle pass.

22.2 Vehicle pass requires: Valid Alaska Vehicle Registration Proof of Insurance (Beginning and Ending dates) Anchorage I/M Emissions Certificate Obtain at Boniface Visitor Control Center.

22.2 Boniface Visitor Control Center, hours of operation are Monday thru Friday 0700 to 1630.

22.3 All contractor personnel require a JBER Identicard.

22.4 Personnel Identicard requires: Contractor to submit Excel file of list of personnel to the contracting officer for approval. Approval by the security forces and may involve background check. Valid Alaska Drivers License and separate photo ID card. Obtain at Boniface Visitor Control Center

23. UNITS OF MEASUREMENT:

23.1 Lump sum items or Each items; Lump sum items or Each items are normally measured for payment as a complete item(s) at the time of 100% completion. When accompanied with an approved progress report, the lump sum item or each item may be paid up to the same percentage as shown on the approved progress report.

23.2 Linear, area, and volume units: Measurements of items by linear, area, or volume may be rounded up to facilitate payment as approved by the project manager as long as the dollar amount of the delivery order is not exceeded.
