PLEASE DO NOT SEND THE COMPLETED QUESTIONNAIRE TO THE CONTRACTOR.
1. The contractor listed below is being considered in a Source Selection by the 338 Specialized
Contracting Squadron, JBSA Randolph, TX. You have received this questionnaire because you have been identified as a source of Past Performance Information for that offeror.
2. Please complete the attached questionnaire for the contractor and program described in Section 1.

RATING DEFINITION
Satisfactory (S)
During the contract period, contractor performance is meeting
(or met) all contract requirements. For any problems encountered, contractor took effective corrective action.
Unsatisfactory (U)
During the contract period, contractor performance is failing
(or failed) to meet most contract requirements. Serious problems encountered. Corrective actions were either ineffective or non-existent. Extensive Customer oversight and involvement was required.
Not Applicable (N/A)
(N)
Unable to provide a rating. Contract did not include performance for this aspect. Do not know.
