About this file
This is a Maintenance Inspection Form used by the City of Phoenix Public Transit Department to document vehicle condition audits conducted on transit buses operated under contract with the City. The form serves as a quality assurance tool to monitor contractor compliance with maintenance standards and safety requirements. Inspections are conducted either randomly or following preventive maintenance activities by the contractor's maintenance staff. The inspection process evaluates numerous vehicle systems and components across multiple areas including the driver's area, interior equipment, passenger areas, accessibility systems, air conditioning, undercarriage, brakes, engine compartment, exterior lights and body condition, and tires and wheels. Each inspected item is marked as passed, defective, corrected, or out of service. The form identifies specific deficiencies requiring contractor correction and establishes compliance timelines, with safety items requiring correction within five days and non-safety deficiencies requiring correction within seven days of inspection.
The form includes provisions for documenting open repair items not completed within the required timeframe and requires the contractor to implement corrective action plans to prevent future occurrences. The inspection is conducted by a City of Phoenix Public Transit Department Quality Assurance Engineer and is addressed to the Contractor's Maintenance Manager. Any bus placed out of service due to safety deficiencies remains unavailable for revenue service until corrective actions are completed. The document serves as an official communication establishing the contractor's responsibility for maintaining all transit vehicles to specified performance and safety standards outlined in the City's contract agreement for fixed-route transit operations and maintenance services, which commenced July 1, 2027 for an initial five-year term with one optional two-year extension.

Date: Unit Number: XXXX
Contractor Maintenance Manager Address.
Phoenix, AZ XXXXX
Dear Mr. XXXXXX:
On (DATE), bus #XXXX, assigned to the City of Phoenix (Location) Transit Facility, was inspected by a Public Transit Department Quality Assurance Engineer as part of the City’s contract monitoring efforts.
As you know, such inspections occur either randomly or shortly after Transdev maintenance staff has conducted regular preventive maintenance activities. During the audit inspection, the following safety items were noted, which resulted in the Quality Assurance Engineer placing the vehicle out of service. As stated in section X.X of the Transdev agreement with the City, all safety deficiencies must be corrected within five (5) days of such inspections. Below is a list of safety items that were noted.
SAFETY ITEMS
DATE OF INSPECTION SAFETY ITEM 5-DAY
COMPLETION DATE
The audit was performed five days following regular inspections and preventive maintenance activities performed by Transdev’s maintenance staff. With the many years of maintenance experience your staff has, it is incumbent upon Transdev to identify such items during its ongoing maintenance activities.
Additional deficiencies identified during the same audit are noted on the Vehicle Maintenance Inspection Form below. As stated in Section X.X of the agreement, all other deficiencies (non-safety) are to be corrected within seven (7) days of such inspections taking place.
Please review the deficiencies identified, focusing on items that did not meet the standards or performance levels outlined in the contract agreement with the city.
Effective immediately, it is (Contractor’s) responsibility to address the issues noted and to implement any plans or procedures that will mitigate similar occurrences in the future.
Sincerely, XXXXXXXXX
Quality Assurance Engineer City of Phoenix
