About this file
This is an Exhibit L Deliverables document prepared by the Texas Department of State Health Services (DSHS) for RFO No. HHS0014612v2 related to Take Charge Texas (TCT) Support Services. The deliverables outline a comprehensive set of project management, transition, and operational requirements for the selected contractor. Key deliverables include a Project Kickoff Presentation due within ten business days of contract effectiveness; a Project Plan encompassing transition activities, project schedules, configuration management, support and maintenance procedures, and status reporting due within seven business days; weekly written status reports detailing work progress, issues, and quality metrics; monthly consolidated status reports including service metrics, maintenance scheduling projections covering ninety business days, and ticket reports; a Readiness Assessment Plan and Results Report to confirm user onboarding and operational readiness; a Turnover Plan due within 180 calendar days addressing transition activities, staffing, roles, data management, risk mitigation, and communication protocols; and a System Security Plan submitted within twenty-one calendar days documenting security controls protecting system confidentiality, integrity, and availability. The contractor must maintain current project schedules using Microsoft Project with weekly updates showing sprint completion percentages, variances, and critical path information, with all documentation stored in SharePoint.
The deliverables establish specific quality assurance standards for user acceptance testing, categorizing results as Green (less than 15 percent failed scenarios), Yellow (15 to 25 percent failed scenarios), or Red (exceeding 25 percent failed scenarios or critical issues). The Support and Maintenance Plan must address service request management, production support procedures, outage handling, incident root cause analysis, software updates, and system monitoring processes integrated with existing DSHS standards. Status reports must commence seven business days following contract effectiveness and continue weekly during both the transition phase and operations phase, with monthly consolidated reports due on or before the tenth calendar day of each month. The Turnover Plan requires annual updates on or before January 1st, and the System Security Plan, designated as a living document, must be updated no less than annually as part of the contractor's risk assessment process.
