About this file
This is a Cost Proposal template (Exhibit F) for the Take Charge Texas Support Services solicitation (HHS0014612v2) issued by the Texas Health and Human Services Commission through the Department of State Health Services. The solicitation seeks a contractor to provide comprehensive maintenance and support services for the Take Charge Texas application, including administrative services, service requests, fixes, defects management, and development, modernization, and enhancement (DME) services utilizing Agile/Scrum methodology. The contract structure encompasses three phases: Phase I (Transition) covering a 2-month period with fixed-fee deliverables including project kickoff presentations, project plans, readiness assessments, and system security plans; Phase II (Operations) spanning the initial term of three years plus optional extension terms of two years, with monthly pricing for administrative services, service requests/fixes/defects, and DME services sprints; and Phase III (Turnover) involving delivery and acceptance of turnover plans. The proposal submission deadline is December 8, 2025, at 10:30 a.m. Central Time, with an anticipated contract award in March-April 2026 and contract commencement in June 2026.
Respondents must submit all-inclusive hourly rates and fixed-fee pricing in the provided Excel template without modifications to structure or formulas. Phase II operations include a maximum of 1,500 hours per month for combined service requests, fixes, defects, and DME services sprints across a 34-month initial term and 24-month optional extension term. Supplemental Services pricing is required for specific resource roles including junior, middle, and senior developers/analysts, product owner liaisons, scrum masters, and testers, with separate pricing tiers for the initial term (Years 1-3) and optional extension terms (Years 4-5). Historical compensation data indicates approximately $800,000 per state fiscal year, though the solicitation notes that the System Agency makes no guarantee of volume, usage, or total compensation to any respondent. Cost evaluation will be based on total cost comparison, with the lowest-cost proposal receiving the highest available rating and higher-priced proposals rated proportionally using a specified formula. Respondents must submit the cost proposal only in the original Microsoft Excel Workbook (.xlsx) format; alternative versions, PDF submissions, or edited templates may result in proposal disqualification.

Notes:
1: This Data tab is used by the Procurement Financial Services analyst to enter static or controlled data in the cost proposal. This method is used to ensure that all CP tabs have the same data where necessary and allows for one change to update many cells in the CP.
2: The yellow shaded cells are used by the PFS analyst to provide basic solicitation information and the Total term, transition period and initial term durations. This data is used to calculate the Phase, Term and Month data used in the CP worksheets.
3: The Title name of each worksheet tab is used to populate the worksheet header in each worksheet and the Total Price summary. It is important to have these headers match the Phase titles in the solicitation.
4: You will find "defined names" used frequently in this CP. This provides accuracy in the calculation of values used throughout the CP. Do not change the defined names unless necessary as they may be referred in formulas used throughout the CP.
