About this file
This document is a Quality Assurance (QA) procedure document for the Critical Utilities Infrastructure Revitalization (CUIR) project at SLAC National Accelerator Laboratory. The document provides detailed descriptions of quality clauses and requirements applicable to subcontractors and suppliers performing work under CUIR contracts. Document CUIR-QA-0175 was originally released on August 27, 2025, and serves as a template and reference guide for integrating quality requirements into Statements of Work (SOW).
The document consists of three primary appendices. Appendix A outlines a template of general QA requirements that may be integrated into SOWs for suppliers and subcontractors, requiring compliance with DOE Order 414.1E, applicable industry standards (such as ISO 9001), and flowed-down quality requirements. Appendix B functions as a checklist identifying 48 specific quality requirements and deliverables (QA-01A through QD-22) that can be selectively applied based on project needs, with the Design Manager, Project Manager, and Quality Assurance Manager responsible for approving applicable selections. Appendix C provides detailed descriptions of each quality requirement and deliverable, encompassing organizational structure, personnel qualifications, design controls, procurement document processes, inspection and testing protocols, nonconformance reporting, corrective action procedures, quality records management, and specialized deliverables including engineering drawings, as-built drawings, material test reports, certificates of compliance/calibration/analysis, audit reports, and ISO 9001:2015 certification documentation. The requirements emphasize a graded approach commensurate with project risk, supplier evaluation and selection, traceability of materials and components, control of measuring and test equipment with NIST traceability, prevention of suspect/counterfeit items, and mandatory notification procedures for nonconformances and corrective action requests within specified timeframes (typically 4-15 business days).
