About this file

This document comprises Task Assignment Change Orders (TACO) for Contract Number HW682 between the State of Florida Department of Environmental Protection (FDEP) and Environmental Consulting & Technology, Inc. (ECT), with project management oversight by FDEP Project Manager Karlee Fowler and ECT Contract Manager Mark A. Culbreth. The work involves environmental remediation and monitoring services at the South Beach Cleaners facility (ERIC_4551) located at 3956 South Third Street, Jacksonville Beach, Duval County, Florida. The file includes multiple change order documents (DC018A-01 and DC018A-02), proposal materials, subcontractor quotes, and supporting cost estimates dated from June through August 2024.

The initial Task Assignment valued at $20,271.72 was modified by TACO DC018A-02, which reduced the total compensation to $19,661.22 due to decreased drilling and Investigation Derived Waste (IDW) disposal costs, while extending the completion date from August 2, 2024 to November 1, 2024 (a 91-day extension). The scope encompasses four subtasks: Project Management ($1,211.46), Monitoring Well Installation including four replacement and new wells with utility location surveys ($12,173.06), Annual Natural Attenuation Monitoring Report with groundwater sampling analysis ($6,296.42), and IDW waste manifest and transportation ($590.78). Drilling subcontractors included GPI ($5,032.00), Earth Tech ($6,985.00), and Preferred Drilling Solutions ($5,095.00), with IDW disposal managed through Erwin Remediation ($375.00). Labor costs totaled 73 hours across multiple staff categories with overhead calculated at 125.30% and fringe benefits at 47.40%. Deliverables included well installation field notes, annual NAM reports, and IDW manifests with invoice milestones tied to FDEP approval dates through June 2024.

The performance of BHUG’s services is limited to full and unobstructed access to include but not limited to: mechanical rooms, manholes, hand holes, vaults, meter rooms, telecom rooms, fixtures (plumbing, electrical, communication), dispensers, fenced compounds, tanks and structures. Full cooperation from the on-site personnel is necessary to perform a complete scan. Should access not be provided or it is delayed, this could impact time to safely complete and accuracy of work.

Re: Non-Hazardous Drum Disposal – State of Florida

Mr. Sattler:

The price for the transportation and disposal (T&D) of drums of non-hazardous soil, mud and water located in the State of Florida is listed below.

Non-Hazardous Drums:

Description Rate Unit

False Mobilization (No drum found) $ 85.00 Per Stop

T&D - Non-Haz Soil $ 110.00 Per Drum

T&D – Non-Haz Mud or Water $ 125.00 Per Drum

The prices listed are all inclusive without any additional fees or charges (fuel surcharge, low volume stop fee etc.) and are valid for all regions in the State of Florida except for the Florida Keys. Pricing for sites located in the Florida Keys will be bid on a case-by-case basis dependent on the volume of drums and the location.

Please ensure that all drum lids are properly secured and that drums are on a level surface like the surrounding area (i.e., not on a pallet, etc.). A single False Mobilization fee will apply if our driver is not able to locate any drum on site.

This quote is valid for 180 days.

Respectfully, Robert Donado Erwin Remediation, Inc.
