About this file
This is a Task Assignment (TA) document issued by the State of Florida Department of Environmental Protection (FDEP) under Contract Number HW682. The document authorizes Environmental Consulting and Technology, Inc. (ECT) to conduct groundwater sampling activities at the Jonfor Cleaners site located at 11700-1 San Jose Boulevard, Jacksonville, Duval County, Florida (Project ERIC_4542). The task assignment was executed on November 16, 2022, with a completion date of April 29, 2023. Kelly Crain serves as the FDEP Project Manager, while Mark A. Culbreth represents ECT as the contractor's project manager.
The scope of work requires ECT to conduct annual groundwater monitoring and provide a Natural Attenuation Monitoring (NAM) report. Specifically, ECT will collect samples from 14 monitoring wells and 4 injection wells for volatile organic hydrocarbon (VOH) analysis and deliver a comprehensive Groundwater Monitoring Report with conclusions and recommendations. The work is structured on a fixed unit rate basis with a total task assignment value of $9,719.58. This amount reflects a reduction from ECT's original proposal of $42,358.10 after project scope was revised to include only a single groundwater sampling event prior to soil vapor extraction (SVE) verification testing. Deliverables include the Annual NAM Report due February 23, 2023 ($9,046.38), and Project Management invoicing due April 29, 2023 ($673.20). The contractor must ensure a current site access agreement is in place before initiating field activities.
