Re: Proposal for South Beach Cleaners
Monitoring Well Installation and Annual Natural Attenuation Monitoring Report
3956 South Third Street
Jacksonville Beach, Florida 32250
Facility ID: ERIC_4551
Contract No. HW682
Dear Ms. Shirah:
In the March 2023 Annual NAM report, ECT recommended installation of replacement monitoring wells for MW-5 and MW-10, assessing the viability of MW009, and installing an intermediate depth monitoring well 100 feet northeast of IW001. On May 8, 2023, the Florida Department of
Environmental Protection (FDEP) approved ECT’s recommendation to install the two shallow monitoring wells and the intermediate depth well. In addition, the FDEP requested that a second intermediate depth well be installed adjacent to monitoring well MW005. The replacement shallow monitoring wells will fill data gaps associated with these locations. The new intermediate wells will further assess the extent of chlorinated ethenes.
Environmental Consulting & Technology, Inc. (ECT) has prepared this scope of work to install the additional wells and conduct the annual natural attenuation monitoring (NAM) for the South Beach
Cleaners in Jacksonville Beach, Florida.
SubTask 1 – Project Management
This SubTask consists of the calculated project management labor, based upon 13% of the direct labor costs in this proposal, and includes fringe, overhead, other direct costs (ODCs) and completion fees.
SubTask 2 - Monitoring Well Installation
This SubTask consists of the installation of four additional monitoring wells. The two shallow wells will be designated MW005R and MW010R. The two intermediate depth wells will be designated IW003 and IW004.
Prior to installation of the monitoring wells, ECT will contact the Sunshine One call center to request subsurface utilities be located. In addition, ECT will contract with a utility locate company to have the areas where the new wells will be installed surveyed to make sure there are no subsurface utilities at those locations. ECT will be onsite with Blood Hound, the utility locate company to verify the locations where the wells are to be installed. The utility locate quotes are provided in Subcontractor Quotes
Attachment.
Ms. Ginger Shirah FDEP ectinc.com
ECT staff will mobilize to the site to install the new wells. These wells will be installed as follows:
Shallow replacement wells:
The two shallow replacement wells will be installed to a total depth of 14 feet below land surface and constructed from 10 feet of 1-inch diameter schedule 40 polyvinyl chloride (PVC), pre-packed well screens threaded to approximately four feet of 1-inch diameter well casing. filter pack will be held in place by a stainless-steel screen. The pre-packed well screens are 4 feet long, so two sections of pre-packed well screen will be used per shallow well.
Intermediate depth wells:
The intermediate depth wells will be installed to a total depth of 27 feet below land surface and constructed from 5 feet of 1-inch diameter schedule 40 PVC pre-packed well screen threaded to approximately 22 feet of 1.5-inch diameter well casing. A 20/40 graded silica sand filter pack will be held in place by a stainless-steel screen.
Completion details applicable to all wells:
All newly installed monitoring wells will be completed with an 8-inch diameter, traffic rated, bolt down well vaults and centered in a 2-foot by 2-foot concrete well pad.
Following installation, each well will be developed by pumping until visible fines are no longer visible in the discharge water. In addition, the top of casing elevations will be surveyed relative to the top of casing elevations of existing wells. Drilling quotes are provided in Subcontractor Quotes
Attachment.
Investigation derived waste (IDW) generated from the well installations will include soil cuttings and development water. The IDW will be placed in Florida Department of Transportation (FDOT) approved
55-gallon drums. The IDW quotes are provided in the Subcontractor Quotes Attachment.
The deliverable for this SubTask will be field notes, well construction and development logs, and well permits. Table 1 provides a summary of ECT personnel activities.

SubTask 3 - Annual Natural Attenuation Monitoring
ECT proposes to perform the tasks necessary to complete the natural attenuation monitoring, consisting of collecting depth-to-water (DTW) measurements at 15 monitoring wells, collecting groundwater samples at eight monitoring wells and containerizing the IDW. The sampling schedule includes collecting DTW measurements from monitoring wells the following wells: MW001, MW003, MW004, MW005R (replacement), MW007, MW008, MW010R (replacement), MW011, MW012, MW013, MW014 (new), IW001, IW002, IW003 (new), and IW004 (new). Groundwater samples will be collected from monitoring wells MW003, MW005R (replacement), MW007, MW010R (replacement), MW012, IW001, IW003 (new), and IW004 (new). All groundwater samples will be analyzed for the volatile organic halogens by United States Environmental Protection Agency Method 8260 by a National
Environmental Laboratory Accreditation Program laboratory. The scope of work is detailed in the attached Level of Effort Worksheet.
The Natural Attenuation Monitoring Report will be prepared and provided to the FDEP Project
Manager. A summary of SubTask 3 personnel activities is provided in Table 2.
