This document is a Task Assignment (TA) issued by the State of Florida Department of Environmental Protection (FDEP) to Environmental Consulting and Technology, Inc. (ECT), a contractor located in Orlando, Florida. The task assignment authorizes ECT to conduct annual natural attenuation monitoring (NAM) of groundwater at the South Beach Cleaners facility in Jacksonville Beach, Florida (Duval County, ERIC_4551). The document includes the executed task assignment agreement signed on December 18, 2024, by Mark A. Culbreth (ECT Contract Manager) and Mubeen Darji, P.E. (FDEP Secretary or Designee), effective through May 30, 2025.

The scope of work encompasses three subtasks: Subtask 1 involves project management activities; Subtask 2 consists of conducting annual NAM sampling from 16 monitoring wells, collecting groundwater samples from nine wells, and preparing an Annual NAM Report; and Subtask 3 involves waste manifest documentation for investigative derived waste (IDW) disposal. The total contract value is $8,484.01 on a fixed unit rate basis, allocated as follows: $7,434.48 for the Annual NAM Report (due March 21, 2025), $1,049.53 for Project Management and Waste Manifest (due April 25, 2025), with all deliverables subject to FDEP approval prior to invoice payment. The cost estimate includes 49 total labor hours across multiple staff categories, vehicle rental, laboratory analytical costs, field equipment and expendables, and subcontracted IDW disposal services through Erwin Remediation at $125.00. The task assignment references the underlying Contract Number HW682 and incorporates the Scope of Work as Attachment 1.

Re: Proposal for South Beach Cleaners Annual Natural Attenuation Monitoring 3956 South Third Street Jacksonville Beach, Florida 32250 Facility ID: ERIC_4551 Contract No. HW682

Dear Ms. Fowler:

Environmental Consulting & Technology, Inc. (ECT) has prepared this scope of work (SOW) to conduct an annual natural attenuation monitoring (NAM) groundwater sampling event for the South Beach Cleaners in Jacksonville Beach, Florida.

Subtask 1 Project Management

This Subtask consists of the calculated project management labor, based upon 13% of the direct labor costs in this proposal, and includes fringe, overhead, other direct costs (ODCs) and completion fees.

The project management cost is calculated in the Level of Effort Cost Estimate Form (LOECEF).

Subtask 2 Annual Natural Attenuation Monitoring

ECT proposes to perform the tasks necessary to complete the natural attenuation monitoring, consisting of collecting depth-to-water (DTW) measurements at 16 monitoring wells, collecting groundwater samples at nine monitoring wells and placing all well purge water into a 55-gallon capacity drum as investigative derived waste (IDW). The sampling schedule includes collecting DTW measurements from the following 16 wells: MW001, MW003, MW004, MW005R, MW007, MW008, MW009, MW010R, MW011, MW012, MW013, MW014, IW001, IW002, IW003 and IW004. Groundwater samples will be collected from monitoring wells MW003, MW005R, MW007, MW009, MW010R, MW012, IW001, IW003, and IW004. All groundwater samples will be analyzed for the volatile organic halogens (VOHs) by United States Environmental Protection Agency (USEPA) Method 8260 by a National Environmental Laboratory Accreditation Program (NELAP) laboratory. The SOW is detailed in the attached Level of Effort Worksheet.

As MW009 is located inside the drycleaner, additional time is included to gain access from the current tenant for the sampling of that well.

An Annual NAM Report will be prepared and provided to the FDEP Project Manager. A summary of Subtask 2 personnel activities is provided in Table 1.

Ms. Karlee Fowler FDEP South Beach Cleaners – Annual NAM Proposal October 25, 2024 ectinc.com

Subtask 3 IDW Waste Manifest

Since the investigation derived waste (IDW) was characterized in 2022 as not impacted by PFAS, ECT will coordinate the transportation and disposal (T&D) of the IDW using the appropriate standard non-hazardous disposal rate. ECT will submit copies of the disposal manifest to the FDEP as the deliverable for Subtask 3. A summary of activities to be conducted under Subtask 3 is provided in Table 2.

Table 2. Subtask 3 IDW Activity Summary Employee/Resource Activity Time Sr. Scientist Characterization and profile of IDW waste 1 hour

Quotes were obtained for transport and disposal (T&D) of the IDW generated at the site, estimated to be one 55-gallon capacity drum. The quotes are summarized below and are provided as an attachment to this proposal. Based on these quotes, Erwin Remediation will be subcontracted to dispose of the IDW generated at this site.

Subcontractor Cost Erwin Remediation $125.00 Perma-Fix $295.75 Moran Environmental $1,195.00

Deliverable and Invoice Schedule

An LOECEF is attached, providing a cost summary for the proposed scope of work. A summary of the deliverable due dates and the invoice schedule is provided in Table 3.

The 2023 annual NAM groundwater sampling event was conducted on November 20, 2023; therefore, ECT intends to schedule the 2024 sampling event in November 2024, once this proposal is approved and the FDEP issues a task assignment.

Should you have any questions regarding this proposal, please contact the undersigned.
