Evaluation Matrix Summary - BPM007036 Community Accountability Program Services for Community Corrections

This document is an Initial and Revisional Consensus Evaluation Matrix prepared by the Arizona state government for the Community Accountability Program Services for Community Corrections procurement (BPM007036). The evaluation assessed six initial bidders—New Freedom Ops LLC, D&D Care LLC, Sage Counseling INC, Prodigy Healthcare LLC, Community Network and Fellowship Inc, and Beia's Families—across three evaluation categories: Experience and Capacity of Offeror (420 points), Method of Approach (400 points), and Cost (180 points), for a total of 1,000 available points. The revised evaluation narrowed the competition to three finalists: New Freedom Ops LLC, Sage Counseling INC, and Prodigy Healthcare LLC. The evaluation criteria assessed company profile and organizational background, company experience with specialized populations including sex offenders and career criminals, financial accounting and stability, specific cognitive behavioral therapy (CBT) methodologies for sexual offense and career criminal populations, office location requirements across multiple counties, statewide telehealth capabilities, 72-hour enrollment requirements, comprehensive program descriptions, staff qualifications, and reporting requirements.

In the final revisional evaluation, Sage Counseling INC achieved the highest combined score of 915 points (385 in Experience and Capacity, 350 in Method of Approach, and 180 in Cost), demonstrating 27 years of business operations, certified staffing in evidence-based curricula, financial stability, 14 operational locations across six counties, and detailed implementation experience with six distinct treatment tracks currently used in Arizona Department of Corrections, Rehabilitation and Reentry (ADCRR) settings. New Freedom Ops LLC and Prodigy Healthcare LLC each scored 750 and 781 points respectively in the initial evaluation. Pricing proposals ranged from $539.25 to $719.00 in the initial round, with revised pricing ranging from $539.25 to $696.00. The evaluation noted that Community Network and Fellowship Inc and Beia's Families received zero points due to missing documentation or failure to meet solicitation requirements.
