“Attachment - Experience and Capacity Response” that demonstrates their understanding of the Scope of Work requirements and describes the company’s overall experience and capacity in providing the Services stated in this Solicitation.

1.0 Company Profile. The Offeror must include a narrative description of its organization.

The narrative must include the following:

Offeror Response:

1.1 Brief overview of business operations;

SAGE Counseling, has read and agrees to comply with all terms, conditions, and requirements in the Scope of Work, Special Terms and Conditions, Addenda A and B to the Special Terms and Conditions, Uniform Terms and Conditions, Exhibits A and B, and other relevant documents from this solicitation, including #29 in Special Terms and Conditions regarding the Department becom-ing a “Smoke-free Facility” as of September 1, 2025.

SAGE was founded in 1998, and our key focus has always been to provide treatment for people involved with the criminal justice and child welfare systems. We have consistently grown over the past 27 years and currently have over two hundred and thirty employees, 14 Outpatient Treatment Centers that are licensed by the Arizona Department of Health Services (9 of which are in Mari-copa County), and facilitate over 320 outpatient counseling groups per week. We are contracted with the Arizona Healthcare Cost Containment system (AHCCCS) and all of Arizona’s AHCCCS Managed Care Organizations/Health Plans, which allow most of our clients to obtain services without any out-of-pocket expenses. SAGE Counseling Inc. is a Sub-Chapter S corporation.

We have included 2 maps below: one of our 9 Maricopa County clinics and the other of our 14 clinics throughout Arizona.

Arizona Offices:

Throughout our history, we have focused on serving both misdemeanor and felony-level clients, most of whom are referred through Community Supervision organization, courts, or prosecutor’s offices. Most of our clients are sent to SAGE through formal contracts for services that we were awarded through a procurement process. Since our beginning, our programs have included sub-stance abuse, DUI services, and domestic violence offender treatment. Our services expanded to include Anger Management, Cognitive Restructuring, Parenting, reentry from incarceration, in home services for families involved with the Department of Child Safety, job skill training, and shoplifting. We provide treatment and education in a variety of settings, including outpatient clin-ics, prisons, jails, client homes, and telehealth services. The majority of groups are held in the evening and on weekends, but we also have morning and mid-day groups available.

We are proud of the fact that since the agency was founded 27 years ago, when a contract under which we had been providing services has exhausted all its renewals, and was re-issued through the formal solicitation process, we were re-awarded the contract each time we submitted a new proposal, except for once. We have received 42 contract re-awards from 43 RFP proposals in

ADCRR Community Accountability Program Services this situation. We believe this track record demonstrates the quality of our programs and that our partners and clients have been highly satisfied with the services we provide.

SAGE has served tens of thousands of criminal offenders from Arizona Department of Corrections Rehabilitation and Re-Entry (ADCRR), the Maricopa County Adult Probation Department (MCAPD), the Maricopa County Attorney’s Office, the Maricopa County Sheriff’s Office, the Mar-icopa County Pre-Trial Services Department, the Maricopa County Superior Court, multiple mu-nicipal courts and prosecutor’s offices, county justice courts, the Department of Child Safety, and other governmental agencies.

We are licensed by the Arizona Department of Health Services (ADHS), Bureau of Medical Facil-ities Licensing (BMFL) and are approved to provide a wide range of behavioral health services.

The agency and provider requirements applicable to SAGE Counseling are described in Arizona Administrative Code (AAC), Title 9: Health Services R9-10, Article 1, General, and Article 19.

These rules and regulations are audited and enforced by ADHS through an initial application and approval process, followed by ongoing auditing activities that require SAGE to demonstrate com-pliance with the applicable regulations. In addition to the counseling and assessment services allowed by an Outpatient Treatment Center, SAGE is approved by ADHS to provide DUI screen-ing, DUI education, DUI treatment, and misdemeanor domestic violence offender treatment ser-vices.

Structure

At SAGE, we make every effort to support and retain our dedicated and highly qualified staff members. We currently employee 68 clinicians throughout Arizona, 23 of whom are licensed by the Arizona Board of Behavioral Health Examiners. SAGE also has 16 program management and clinical supervisory staff.

One of the many unique components of SAGE is our administrative structure. SAGE employs 35 administrative staff which includes administrative assistants, client service associates, and recep-tionists. They support our clients and staff and directly coordinate with the referral partners.

In our outpatient clinical programs, we initiate services by administering an industry-leading risk-of-recidivism instrument, the Ohio Risk Assessment System (ORAS). It identifies clients’ highest criminogenic needs and enables us to conduct Treatment Matching and place clients into the most appropriate Treatment Track. The duration and dosage of treatment are also assigned based on the assessment. Following Evidenced-Based Practices, most of our Treatment Tracks are gender-specific (including all of the Substance Abuse Tracks), and we keep low-risk to recid-ivate clients separate from moderate or high-risk ones.

Counselors’ General Training

All newly hired clinicians receive training in the following topics and skills:

1. Providing group counseling.

2. Engaging clients in groups.

3. Managing difficult or oppositional clients.

4. The primary criminogenic factors.

5. Working with criminal justice clients.

5. Facilitating counseling groups in a virtual environment, and

6. Adhering to SAGE policies regarding clinical services.

7/25/2025 5 | P a g e

Clinicians are trained separately in each curriculum we use, and they must complete the whole training regimen prior to facilitating a group on their own. Initially, our staff observe counseling groups facilitated by lead counselors, then co-facilitate a group with their clinical supervisor. They are also directly observed by their supervisor for at least their first two independent groups.

We provide cultural competency and diversity training for all our staff, and we encourage the inclusion of clients’ religious and cultural preferences. Additionally, SAGE’s counselors partici-pate in a broad training program through our enterprise eLearning system, Relias, which is pro-vided by AHCCCS, Arizona’s Medicaid program. Among the required clinical classes for coun-selors are:

• Cultural Competency in Health Care

• Quality of Care Concerns

• HIPAA: Privacy Rule

• HIPAA: Basics

• AHCCCS - Health Plan Fraud

• Supporting Client Rights in Behavioral Health

• Preventing, Identifying, and Responding to Abuse and Neglect

• Ethics and Corporate Compliance

Assessor Training

SAGE’s assessors begin their training by observing/shadowing 5 assessments that are completed by lead assessors. Next, they meet with the Assessment Team Supervisor and begin the formal training process, which includes the following topics:

1. Learning each of SAGE’s programs and contracts’ specific requirements, e.g. Substance Abuse Treatment, Prison Transition/Re-Entry Program DV, Anger Management, Shoplift-ing Diversion Program, etc.

2. Training on all the screening assessment instruments we utilize, including the American Society of Addiction Medicine’s (ASAM) Continuum, TCU Drug Screen 5, TCU Criminal Thinking Scales, Patient Health Questionnaire (PHQ-9), Generalized Anxiety Disorder-7 (GAD-7), Social Determinants of Health (SDOH), Domestic Violence Severity Index-R, etc.

3. Two days of training on the Ohio Risk Assessment System (ORAS) and subsequent test-ing. Certification to administer the tool requires passing the test. 5 more days of training are required to become a Certified Trainer of the ORAS. We periodically conduct ongoing training to enhance assessors’ administration and scoring skills.

4. Conducting collaborative interviews and making effective clinical decisions.

5. Diagnosing effectively and with the Diagnostic and Statistical Manual 5th Edition, Text Re-vision (DSM 5 TR).

6. Developing Individualized Service Plans (ISPs), and

7. Maintaining the Golden Thread across the assessment findings, diagnoses, and ISP.

When new assessors are evaluated as being ready to conduct assessments with clients, they complete 3 to 5 assessments under the direct monitoring of the Assessor Supervisor or Man-ager. When they have demonstrated competence with all the protocols and tools, they begin completing a small number of assessments independently. The number of clients they assess in a day is gradually increased in tandem with their proficiency. Our comprehensive assessments

ADCRR Community Accountability Program Services are scheduled for 90 or 120 minutes and fully meet all of AHCCCS’ and ADHS/BMFL’s require-ments.

Case Manager Training

Initial training for SAGE Counseling’s case managers is a 2-week training process provided by the supervisor of case management team. The initial week entails general case manager duties, responsibilities, and skill development, including:

• Review of SAGE’s policy and procedures.

• Documenting services in Credible, our electronic health record.

• Client resource location and navigation.

• AHCCCS billing standards.

• Linking clients’ services with their individual service plan (ISP) goals.

• Updating clients’ ISPs as appropriate, and

• Case management services specifically for justice-referred adults.

The second week of training focuses more on the needs and requirements of the case manager’s specific case load, as well as the requirements of the related referral sources, e.g. Domestic Vio-lence and DUI offenders and ADCRR or MCAO. The primary areas of training include:

• The clinical interventions SAGE uses for its clients’ programs, e.g., the ORAS, multiple curricula, curriculum exercises and homework, Relapse Prevention Plans, etc.

• Common ISP goals and objectives for these clients.

• Common resources that are often needed by these clients.

• Coordination of care both internal to SAGE and making referrals with external resources.

• Utilizing the Unite Us platform for care coordination and referrals.

Case managers then begin shadowing lead case managers, and shortly thereafter have clients gradually assigned to their own caseload.

SAGE is in good standing with the Arizona State Government. 2.0 Company Experience

2.1 What experience do you have serving clients within a corrections environment?

Offeror Response:

SAGE Counseling Inc.

SAGE Counseling, Inc. was developed in 1998 to provide clinical and educational services for criminal justice organizations, the child welfare system, and their referred clients. We are locally owned and managed, and our key focus has always been to provide treatment for people involved with the criminal justice system.

In addition to having a broad range of experience with criminal justice organizations and clients, SAGE has specifically provided services in incarcerated settings since 2002, including in both prisons and jails, with adults and juveniles, and with males and females. We currently have 7 staff completing services within various jails around Arizona as well as 2 staff that currently work with the Maricopa jails population with similar behavioral health programing. A list of relevant contracts has been included below along with the type of treatment that is conducted for each contract.

Following EBP, our treatment approach is primarily Cognitive-Behavioral in orientation, and we consistently employ Motivational Interviewing techniques, the Stages of Change Model, a Strengths-Based Orientation, and Trauma-Informed Care. These are the orientations that have been shown to be most effective with a wide variety of criminal offending populations, and so they are the primary treatment methodology employed at SAGE Counseling. All our programs em-phasize personal responsibility, increased awareness of problematic beliefs and behavior, and key skill development and rehearsal. Our clinical interventions cultivate realistic opportunities for change by addressing dynamic criminogenic needs which reduce recidivism.

SAGE Counseling’s treatment programs are grounded in the evidence-based philosophy that in-tervention with justice-involved clients is most effective when it is:

1. Founded on evidence-based interventions and practices

2. Matched to clients’ specific risk and need level

3. Based on thorough assessment procedures

4. Targeted toward multiple dynamic criminogenic needs

5. Oriented in a primarily cognitive-behavioral approach

6. Couched in Motivational Interviewing and The Stages of Change Model

7. Provided by well-trained and well-supervised counselors

8. Based on highly structured curricula developed for that specific population.

One of the key leaders and researchers in the field of substance abuse treatment, Thomas McLel-lan, Ph.D., has emphasized that effective treatment is based on the scientific principles of the National Institute on Drug Abuse in “Principles of Drug Addiction Treatment: A Research-Based Guide” (McLellan, p. 17), whose central components are:

1. No single treatment is appropriate for all individuals.

2. Effective treatment attends to multiple needs of the individual, not just drug use.

3. Remaining in treatment for an adequate period of time is critical for treatment effectiveness.

7/25/2025 31 | P a g e

ADCRR Community Accountability Program Services

4. Counseling (individual and/or group) and other behavioral therapies are criti-cal components of effective treatment for addiction.

5. Medications are an important element of treatment for many clients.

For over 25 years, our general goals in our treatment program for offenders have included:

▪ Eliminate criminal activity and illegal substance use

▪ Reduce victim-stancing and increase clients’ responsibility for their behavior

▪ Teach clients to change their beliefs and attitudes regarding excessive enti-tlement, blaming others, and thinking errors that support illegal behavior

▪ Improve self-efficacy concerning the ability to make self-directed, positive changes

▪ Increase self-awareness, particularly regarding emotional states and personal triggers

▪ Increase key personal skills, e.g., refusal skills, conflict resolution, anger man-agement, problem-solving, impulse modulation, stress management, and so-cial skills.

SAGE currently facilitates cognitive restructuring, anger management, domestic violence treat-ment, parenting, inmate employment skills building educational services, substance abuse inmate educational services and individual counseling for juveniles adjudicated as adults in the jails through the Maricopa County Sheriff’s Office (MCSO). SAGE has held contracts with the MCSO to provide clinical service in the jails since 2000.

In addition to the Maricopa County Sheriff’s Office (MCSO), we also have contracts to provide cognitive restructuring groups for:

• The Maricopa County Attorney’s Office

• The Arizona Department of Corrections Rehabilitation and Re-Entry, and

• The City of Phoenix Prosecutor’s Division.

Since our inception, SAGE has also served thousands of criminal offenders who were referred by the Maricopa County Adult Probation Department (MCAPD), the Arizona Department of Correc-tions Rehabilitation and Re-Entry, the Maricopa County Superior Court, multiple municipal and justice courts, the Department of Child Safety, and other governmental agencies.

All of SAGE’s programs have a Cognitive-Behavioral Treatment (CBT) platform and are based on the principle that thinking drives behavior. In order to change problematic behavior, we help clients change their thinking. We target functional treatment goals such as recovery from sub-stance abuse, proactively responding to high-risk situations, problem solving, social skill develop-ment, refusal skills, fostering and maintaining healthy relationships, positive alternatives to ag-gression, maintaining employment, and clarifying personal values. Our counseling groups also teach and rehearse key skills for clients to reduce criminal thinking and behavior, including skills for anger management, assertiveness, communication, decision-making, and using positive self-talk.

Our interventions are focused and structured, emphasizing client accountability, specific skill de-velopment, resolving criminogenic needs, pro-social thinking and behavior, and reducing exces-sive entitlement. We are committed to continually improving our programs according to Evi-denced-Based Practices to realize better outcomes with those we serve.

ADCRR Community Accountability Program Services

SAGE’s counselors are highly skilled in positively engaging our clients, and we often receive phone calls and emails from our clients and referral sources who tell us how much they appreciate the dedication and caring of our staff. We have developed a comprehensive infrastructure that enables us to efficiently provide the kind of clinical services and accurate tracking that the criminal justice system requires.

SAGE has been a Medicaid/Arizona Health Care Cost Containment System (AHCCCS) provider since 2018, and we are currently credentialed with all AHCCCS Managed Care Organizations.

Being able to accept AHCCCS health plan coverage for clients’ services has removed the single largest barrier for criminal justice participants completing treatment—the cost. For those who qual-ify, AHCCCS covers nearly all the services that SAGE provides, as they are almost always med-ically necessary.

We provided a list of SAGE Counseling’s general experience providing substance abuse and cognitive restructuring treatment to individuals referred by the criminal justice system.

Arizona Association of Health Plans (AzAHP)

The members of the Arizona Association of Health Plans (AzAHP) are the companies that pro-vide health care services to more than two million Arizonans enrolled in the Arizona Health Care Cost Containment System (AHCCCS).

We are the private half of the public-private partnership model that makes AHCCCS one of the most successful Medicaid managed care programs in the United States. The AHCCCS program works – and protects taxpayer dollars – because of our ability to effectively leverage the private sector to offer AHCCCS members high-quality care from more than 100,000 Arizona healthcare providers.

AzAHP works on issues of collective health plan interest including engaging at the Legislature and with the Executive Branch. We also provide resources and expert advice to policymakers on the value our member health plans bring to the AHCCCS program and Arizona’s health care delivery system.

AzAHP also makes it easier for providers to join a health plan network through the Credentialing Alliance as well as supplies assistance, training and resources to enhance the workforce across all lines of business through our Workforce Development Alliances.
