About this file
This is an Annual Procurement Disclosure Statement (SPO Form 110) filed by Michael Percival, Bureau Administrator at the Arizona Department of Administration, Community Corrections Reform (ADCRR), effective for Fiscal Year 2025 and signed on March 14, 2025. The form serves as a conflict of interest disclosure statement required under Arizona Revised Statutes for all public officers and employees with significant procurement roles. Percival acknowledges his responsibility to comply with state ethics and procurement statutes (A.R.S. 38-501 to 38-511, 41-741, 41-753, 41-2503, 41-2517, and 41-2616) and confirms awareness of standards governing personal influence, financial interests, and official decisions related to state contracts. The disclosure requires listing of all ownerships, employments, and affiliations held by Percival and his relatives that may create substantial pecuniary or proprietary interests in any contract, sale, purchase, or service involving the agency.
Percival attests through multiple binding statements that he has not and will not communicate with potential vendors regarding confidential procurement information, specifications, or evaluation tools that would provide unfair competitive advantages; has not provided insight or assistance to any offeror that could inhibit fair competition; and bases all solicitation input solely on legitimate state requirements. He further declares no known conflicts of interest, commits to immediate disclosure should conflicts arise, and confirms he will not receive direct benefits from confidential information he prepared or assisted in preparing. The form specifies that these provisions remain in effect following employment termination until all associated solicitations are awarded and continue to apply if Percival accepts employment with any entity that may submit offers to relevant solicitations. Violation of these terms subjects Percival to suspension for not less than ninety days or dismissal from state service. No substantial interests requiring disclosure were listed on the form.
