About this file
This is a General References form designed for offerors to provide the Government with information about their key business relationships and financial partners as part of a federal contract proposal submission.
The form requires offerors to identify and provide contact information for four categories of references: (1) Surety company, including the surety's name, address, agent name, and telephone number; (2) Corporate banks (two entries), with details on the bank name, address, agent contact, telephone number, and types and numbers of accounts maintained; (3) Insurance company, including the provider's name, address, agent name, telephone number, and types of policies; (4) Subcontractors (three entries), requiring the subcontractor's name, address, contact name, telephone number, and area of specialized work; and (5) Material suppliers (three entries), with the supplier's name, address, contact name, telephone number, and types of materials provided. The form emphasizes that offerors must disclose instances where relationships with references may be considered less than fully satisfactory, provide explanations of remedial corrective actions taken or planned, and include area codes with all telephone numbers. Failure to candid disclosure may result in an unfavorable assessment of the offeror's past performance record. This is a template form without specific contract details, response dates, or pricing information.

General References READ CAREFULLY: The purpose of this form is for the offeror to provide the Government with general references in the spaces provided as described in the Instructions, Conditions, and Notices to Offerors (Section L). Include Area Codes with all telephone numbers. Fill in the information by typing or printing legibly.
It is especially important that the offeror disclose instances in which their relationships with the references may be considered less than fully satisfactory. The offeror should tell their side of the story and describe remedial corrective action that has been taken or will be taken to correct the deficiency. Failure to do so may result in a determination that an offeror has been less than candid with the Government, which could result in an unfavorable assessment of the offeror’s past performance record. Provide explanations on separate sheets as necessary.
