About this file

This document is a collection of Operational Control Documents for construction contractor environmental health and safety compliance at the Army Medical Center (AMP-400) facility in Oklahoma City.

The document establishes baseline operational control procedures across six environmental and occupational safety and health (OSH) aspects: (1) Storm and Surface Water Discharges (AMP400-ENV-OC-2.4.3-CC), requiring compliance with OPDES permits, City of OKC ordinances, and MMAC Stormwater Pollution Prevention Plan through contractor training and site inspections; (2) Solid Waste Generation (AMP400-ENV-OC-2.5-CC, Revision 1.0), targeting reduction of landfill waste through waste reduction and recycling with monthly reporting to AMP-100 for projects exceeding $100,000; (3) Toxic Substance Releases (AMP400-ENV-OC-2.9-CC), addressing asbestos, lead paint, and PCB light ballast discovery and disposal during demolition and renovation activities; (4) Electrical Safety (AMP400-OSH-OC-3.1-CC), requiring personnel training appropriate to electrical hazard exposure levels and implementation of lockout/tagout procedures; (5) Falls Protection (AMP400-OSH-OC-5.1-CC), mandating training on fall hazards, ladder inspections, and scaffold qualifications; and (6) Physical Hazards (AMP400-OSH-OC-8.1-CC, Revision 1.0), requiring appropriate personal protective equipment and worksite inspections to address sharp edges, flying debris, crushing hazards, and power tool safety. Each document designates responsibility to the Construction Contractor or Site Supervisor for implementation, with AMP-400 Inspectors providing contract support and oversight. Documentation requirements include contractor training certifications, advisory notices for non-compliance, and mishap reports submitted to AMP-100.
