About this file
This is an instructions document for the Key Staff Qualifications Form (Attachment 16) issued by the State of California Department of Technology for RFP 26-014-280. Bidders must submit a separate Key Staff Qualification Form for each cited project used by key staff members to meet mandatory or desirable qualifications. A maximum of five Key Staff Qualification Forms per proposed key staff role is permitted, with each form accompanied by a corresponding Key Staff Reference Form (Attachment 17). All experience must have occurred in the United States within the twelve years prior to the solicitation release date. The document defines key terms including "large-scale projects" as those with over 1,000 end users involving significant budgets and resources spanning multiple years, and "completed implementation" as solutions that have received system acceptance and are in production environments. Dates must be submitted in MM/DD/YYYY format, and partial-year experience for desirable qualifications must be rounded down.
References cited on the Key Staff Qualification Forms must be from the company or government agency for which the project was developed, must hold a management or supervisory role on the referenced project, and must be external to the bidder's organization. References cannot be current State employees who are members of the Evaluation Team, State Water Resources Control Board Division of Drinking Water or Information Technology employees, or State contractors. Only one reference from the California State Water Resources Control Board is permitted for this solicitation. Bidders must clearly document how each key staff member's experience meets the minimum qualification components of each mandatory experience requirement, as the State will deem proposals non-responsive if it cannot determine that all mandatory qualifications have been met.
