About this file

This document is an instruction file for bidder qualifications forms issued by the State of California Department of Technology for RFP 26-014-280. The instructions establish procedures for bidders to demonstrate their qualifications and experience through the submission of Attachment 14 (Bidder Qualifications Forms) and corresponding Attachment 15 (Bidder Reference Forms). Bidders must submit a minimum of three projects and a maximum of five projects, each on a separate form with matching reference documentation. All cited projects must have occurred within the last twelve years prior to the solicitation release date, with dates recorded in MM/DD/YYYY format. Projects must be calculated using full-time and part-time month equivalents as defined in Section 3.3.5, with partial year experience rounded down. Large-scale projects are defined as extensive efforts with significant budgets and large teams spanning multiple years, while complex IT projects involve multiple interconnected components requiring high technical expertise.

Bidders must have served as the Prime Contractor on all cited projects, with projects defined as individual efforts where the bidder held prime contractor status. Each Bidder Qualifications Form requires documentation of project name, client company name, contact information for a project reference, start and end dates, project description, contract amount, and confirmation that the project is of similar size and scope with at least two elements matching. References must be external to the bidder's organization, hold management or supervisory roles on the referenced project, and be employed by the client organization. Only one reference from the California State Water Resources Control Board (SWRCB) is permitted. If more than five forms are submitted, only the first five in order will be evaluated. Bidders are responsible for ensuring all mandatory qualifications are met and properly addressed in the submitted forms, as proposals lacking clear documentation of experience may be deemed non-responsive.
