About this file
This is a Key Personnel Attachment form submitted to the Arizona Department of Corrections for Solicitation No. BPM007036, which seeks Community Accountability Program Services for Community Corrections Rehabilitation & Reentry. The document identifies three key personnel proposed to support the Community Accountability Program contract, each with distinct clinical and management responsibilities. Brandy Judson (LMSW, Chief Clinical Officer) has 9.5 years of employment with the company and 16 years of government agency experience, dedicating 5% of her time to oversee clinical staff supervision, training, program curricula development, and quality assurance across outpatient programs. Cassidy Brooking-Lang (LCSW, Clinical Director) brings 1.5 years of company tenure with 12 years of government agency experience and will dedicate 10% of her time to clinical supervision, staff training, program development, and contractual compliance. Makenzie Middleton (LAC, Clinical Manager) has 1.5 years of company experience with 5 years of government agency background and will commit 25% of her time to clinical and administrative supervision, staff recruitment and orientation, contract compliance monitoring, and statistical reporting obligations.
All three key personnel hold relevant clinical licensures (LMSW, LCSW, and LAC respectively) and demonstrate substantial experience in community corrections and mental health service delivery. The proposed staffing structure establishes a tiered clinical leadership hierarchy designed to ensure high-quality, client-centered services with appropriate oversight of clinical fidelity, staff performance, and contract compliance. Each individual's proposed responsibilities align with program management, quality assurance, and direct clinical supervision requirements typical of community-based rehabilitation and reentry initiatives. The document requires that all responses address the key personnel's experience with similar services, government agency work history, proposed time allocation, and supporting documentation through separate resumes, which are treated as supplemental materials to the completed form.
