About this opportunity
The County of Santa Clara Social Services Agency's Department of Aging and Adult Services seeks qualified vendors to provide Community Living Connection (CLC) services under RFP-SSA-FY26-0522. This is a case management and supportive services procurement designed to enable high-risk older adults (aged 60 and older) and adults with disabilities to continue living independently within Santa Clara County. The program delivers comprehensive case management through four service phases: intake and screening, connection and assessment with home evaluations, intensive case management with housing navigation for complex cases, and moderate case management with housing navigation for stabilized clients. Additional service components include purchase-of-services arrangements, transportation assessments, staff training, multidisciplinary team coordination with other County departments, and participation in a shared referral and outcome tracking system. Eligible clients must be Santa Clara County residents at or below 300 percent of the Federal Poverty Level who are willing to participate and demonstrate need to prevent hospitalization or enable community living. Case management staff must hold a Master of Social Work (MSW) degree or work under MSW supervision. Vendors will be evaluated on staffing qualifications (10 points), organizational capacity (10 points), language proficiency plan (10 points), program narrative (40 points), data collection and outcome measurement (10 points), budget and budget narrative (10 points), past performance (5 points), and local preference (5 points), for a total of 100 points. A non-mandatory pre-proposal conference is scheduled for February 3, 2026, at 10:00 AM PST via Microsoft Teams. Written questions must be submitted by February 10, 2026, with responses provided by February 17, 2026. Proposals must be submitted electronically through Biddingo by March 10, 2026, at 3:00 PM PST as a single PDF in Arial 12-point font, single-spaced, with a 40-page maximum for narrative content. Finalist selection is anticipated for April 10, 2026, with contract negotiations scheduled for April 21-28, 2026, and notices of intent to award issued by May 5, 2026.
The RFP is open to private nonprofits, educational institutions, public agencies, and other eligible organizations that are not suspended or debarred from federal procurement. A local preference of 5 points is available for Santa Clara County-based businesses that demonstrate meaningful production capability within the county; post office boxes, residential addresses, or local sales offices without support cannot serve as the sole basis for this designation. Vendors must submit current independent audited financial statements and management letters dated no more than eighteen months prior to the proposal due date and must demonstrate administrative and fiscal capability in accordance with Generally Accepted Accounting Principles (GAAP). Vendors must also disclose any history of corrective action notices, contract cancellations, controlling interests in other organizations, litigation involving principals, or business associations within the preceding five years. The procurement may result in a single or multiple contract awards with an estimated total budget allocation of $400,000 per one-year contract term. The initial contract term runs from July 1, 2026, through June 30, 2027, with the County retaining the option to renew for up to four additional one-year extensions based on satisfactory performance and funding availability. Compensation is available on either a cost-reimbursement or fee-for-service basis for actual, reasonable, necessary, and allowable costs, with administrative overhead for cost-reimbursement contracts capped at 15 percent of available funding. Vendors must comply with nondiscrimination laws, wage and hour regulations, living wage requirements for contracts exceeding $100,000, the California Public Records Act confidentiality process, and County conflict-of-interest and confidentiality requirements. Contractors must maintain comprehensive insurance coverage including general liability ($1,000,000 per occurrence/$2,000,000 aggregate), automobile liability ($1,000,000 combined single limit), and workers' compensation, with the County named as an additional insured.
