About this file
This document is Appendix D of Request for Proposal RFP-SSA-FY26-0522 issued by the County of Santa Clara Social Services Agency's Department of Aging and Adult Services. The appendix provides instructions for vendors submitting proposals for the Community Living Connection program, which aims to provide case management and other services to high-risk older and disabled adults to enable independent living. The RFP may result in single or multiple contract awards with an initial term of one year and an option for the County to renew for up to four additional years.
The document outlines the California Public Records Act (CPRA) confidentiality request process, requiring vendors to identify specific page numbers and sections of their proposals they wish to keep confidential and provide a legal basis under Government Code Section 6254 for any exemption request. Vendors are explicitly prohibited from marking their entire proposal as confidential or proprietary; only clearly identified sections with valid legal justification will be considered for exemption from public disclosure. Vendors may alternatively indicate that they are not requesting any confidentiality exemptions by checking the provided box.
