About this file
This is an appendix containing vendor experience and administrative fiscal capability requirements for the Community Living Connection Request for Proposal (RFP-SSA-FY26-0522) issued by the County of Santa Clara Social Services Agency, Department of Aging and Adult Services. The RFP seeks qualified vendors to provide case management and related services enabling high-risk older and disabled adults to maintain independent living. The County intends to award a contract with an initial one-year term and the option to renew for four additional years, unless earlier terminated or amended. The document does not specify response deadlines, site visits, bidders' meetings, or award dates.
The appendix establishes mandatory administrative and fiscal capability requirements that vendors must meet to qualify for consideration. Vendors must demonstrate private nonprofit, educational institution, public agency, or other eligible status; confirm they are not suspended or debarred from federal procurement; and disclose years of service under their present and any former business names. Vendors are required to certify their ability to establish and maintain financial controls in accordance with generally accepted accounting principles, maintain accurate financial records, ensure proper use of program funds, and comply with federal, state, and county statutes and regulations. Additionally, vendors must submit a current independent audited financial statement and management letter from a certified public accountant dated within eighteen months of the proposal due date. The appendix also requires vendors to disclose any previous notices of corrective action, contract cancellations, controlling interests in other organizations, litigation involving principals, and business associations from the preceding five years.
