About this file

This document is a Certification of Work form designed for contractors to document maintenance and service activities. The two-page form provides spaces for contractors to record details such as the building location, date of visit, contractor personnel onsite, and specific work performed, including preventative maintenance services and service calls. The form requires signatures from both the contractor and facility manager to certify that the listed work was completed in accordance with contract requirements and to the best of their knowledge. The form is intended to be saved in a Contract Management and Maintenance System (CMMS) for record-keeping and documentation purposes.
