The place of performance will be at contractor and government facilities including Tinker Air Force Base.

SHOP VISIT REPORT REQUIREMENTS
Block 4: Section I- Introduction to include:
a. Report from field of reason for removal of item
b. Condition of item on arrival at repair facility
c. Inspections performed on item by Task Number listed in applicable Technical Data
d. Results of inspections on item stating the electrical and physical test requirements and parameters which the item does not meet
e. Use photographic evidence to support non-conforming material.
Section I-Summary to include:
a. Price and repair time to return item to serviceable condition; Proposed and actual repairs performed on item to return to serviceable condition
b. A list of all complied service actions
c. A list of any replaced serialized component
Section I Conclusions to include:
a. Actual repairs performed on item to return to serviceable condition by Task Number listed in the applicable Technical Data
b. Results of performance tests following repair completion
