ATTACHMENT L
SAFETY REPORT
Block 4: 2. Format. The SAR shall be presented in the style as defined in Propulsion Center of
Excellence's (PCoE) Best Practice (BP) 99-06E (see SOW Attachment X – Flight Safety Risk
Management Process), AFMAN 20-116, and Mil-Std-882E. The Contractor shall provide a narrative and the method by which the safety issue was discovered.
3.2.b. A brief historical summary of the safety and reliability of the affected system/component driving the risk.
3.4.a. Description of methodology to accomplish the risk analysis (e.g. Weibull, Monte Carlo, Log Normal).
3.4.b. The resulting determined Baseline Risk as measured against the NRIFSD and ERLOA events thresholds established in AFMAN 20-116.
3.5.a. A recommended solution/mitigation to reduce the baseline risk to an acceptable level in accordance with the NRIFSD and ERLOA events thresholds established in AFMAN 20-116. The solution/mitigation shall concentrate on an integrated systems engineering approach to provide a revised product that eliminates the failure conditions experienced in field operational usage.
Interim, Corrective Action, Target, and Cumulative risk shall be identified. The results shall further be presented in the format identified in PCoE BP 99-06E Appendix B.
